Risk factors associated with the conversion of meticillin-resistant Staphylococcus aureus colonisation to healthcare-associated infection

本研究の目的は、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の長期保菌患者における医療関連 MRSA（HA-MRSA）感染症のリスク因子を特定することである。本研究は地域教育病院で実施された症例対照研究である。対象患者は 18 歳以上かつ MRSA 鼻腔内保菌の培養陽性症例 41 例と対照 82 例であり、前者は保菌の初回確認後 60 日以内に HA-MRSA 感染症を発症した者、後者は発症しなかった者とした。想定されるリスク因子として評価したのは、人口統計学的特性、併存疾患、使用薬剤、侵襲的器材の有無、創傷その他の感染症の有無、栄養状態、入院回数、および感染症発症までの期間であった。単変量解析では、末梢血管疾患、3つ以上の併存疾患、中心静脈カテーテル、フォーリーカテーテル、および 2 回以上の入院が、HA-MRSA 感染症リスクの増加と有意に関連していた。多変量解析からは、中心静脈カテーテル（OR 8.00、95％CI 3.13 ～ 20.4）または2回以上の入院（OR 3.37、95％CI 1.37 ～ 8.26）を MRSA 保菌者における MRSA 感染症の独立リスク因子とするモデルが得られた。結論として、MRSA 保菌から HA-MRSA 感染症への移行と独立して関連するリスク因子は、中心静脈カテーテルまたは 2 回以上の入院であった。この患者集団では、リスク因子を最小化する戦略が HA-MRSA 感染症の減少に有用と考えられる。
