Plan-do-study-act cycles as an instrument for improvement of compliance with infection control measures in care of patients after cardiothoracic surgery

本研究の目的は、715床の大学病院において、心臓胸部手術の術中から術後のケアにおける感染制御対策の遵守率が「計画・実行・評価・改善」（PDSA）サイクルの採用により向上するかどうかを判定することである。ここでいうPDSAサイクルの評価項目は、感染制御スタンダードに基づく正しい手技の遵守率とした。介入は、看護師および医療スタッフに対するPDSAサイクルの採用についての指導と訓練、PDSAサイクルを導入した時点における評価のフィードバック、手術室の周囲にポスターを貼ることなどであった。追跡期間中、全遵守率が向上したのは体外循環技術者が使用する部屋と手術室だけであった。追跡期間終了後の調査では、手術室の遵守率が低下していたが、集中治療室（ICU）長期入室患者の血管内留置カテーテルのケア、および看護病棟入院患者の創傷ケアでは遵守率が向上していた。最終的な一連の調査では、一般的な感染制御策の遵守率が手術室で再び向上し、病棟およびICUでも十分なレベルを維持していたが、手術室からICUへ転室した直後の患者に関しては例外であった。これらの結果から、PDSAサイクルの採用により、感染制御策の遵守率が著しく向上しうることが示された。しかし、向上した遵守率を維持するためには、反復的な調査が必要である。

医療に継続的質改善活動の手法であるPDSAサイクルを導入した点で注目される。なお、PDSAサイクルはPDCA（plan-do-check-act）サイクルとしても知られている。
