Urinary catheterization in care homes for older people: self-reported questionnaire audit of catheter management by care home staff

自己記入式アンケートを用いて、英国の国立最適医療研究所（National Institute for Clinical Excellence；NICE）および失禁ケア協会（Association of Continence Care）が発表した導尿カテーテル処置指針に関する介護施設職員の申告による知識を判定し、さらにカテーテル処置実施率が高い施設では、この知識に相違があるか否かを調査した。カテーテル処置実施率が高率、中程度、または低率の、無作為に選択された37カ所の介護施設から、看護師およびその他の介護職員1,438名中750名（52％）の回答を得た。サンプリングを行った3カ所の健康行政区域の介護施設、あるいはカテーテル実施率が異なる介護施設において、申告された介護内容に差はなかった。看護師の83％およびその他の介護職員の40％は、正式なカテーテルケアの訓練を受けていた。しかし、少なくとも全職員の10％が、カテーテルを取り扱う前に手洗いを行わず、また採尿バッグから排尿する時期は、4分の3まで貯留した時点ではなく、一杯になる時点まで遅らせていると申告した。入所者に自分でカテーテルバッグを空にするよう指導していたのは、看護師45％、他の介護職員40％のみであった。ルーチンでカテーテルのメンテナンス用の溶液を使用または膀胱洗浄を実施していると申告したのは、全職員の50％であった。採尿バッグの栓から尿検査検体を採取していた看護師は29％、その他の介護職員54％であった。指針の遵守は1998年の監査以降、大幅に改善しているが、一部では依然として遵守されていない。地域の監査を継続し、特に資格のない介護職員を対象として尿カテーテルケアの正式な訓練を行う必要がある。地域におけるNICEガイダンスの実施を確実なものにするためには、教育が必要である。
