目的および方法
病院環境(3 次病院 2 施設および 2 次病院 1 施設の表面および空気サンプル)における酵母菌集団を、選択的培養、DNA メタバーコーディング、および新規二重 qPCR アッセイにより評価した。
結果
本調査では、WHO が優先すべきとした 2 種類の日和見病原体であるカンジダ・パラプシローシス(Candida parapsilosis)およびカンジダ・アルビカンス(Candida albicans)が多く認められた。C. parapsilopsis は幅広く病院すべての区域、すなわち屋外から屋内の入院受付エリアまで、病室、および HEPA フィルターで保護された集中治療室(ICU)で認められた一方、C. albicans は特に病室および ICU に関連して認められた。臨床的に重要なその他の酵母菌として報告されたのは、例えばカンジダ・トロピカーリス(Candida tropicalis)、Nakaseomyces glabratus(旧称カンジダ・グラブラータ[Candida glabrata])、Pichia kudriavzevii(旧称 Candida krusei)、クラビスポラ・ルシタニエ(Clavispora lusitaniae[旧称 Candida lusitaniae])、Meyerozyma guilliermondii(旧称 Candida guilliermondii)および Lodderomyces elongisporus であった。
結論
本研究は、医療環境における環境内の微生物アセスメントを改善するための重要な知見を提供している。こうしたアセスメントは、適切な室内環境を保証し、院内感染を予防する上で極めて重要である。
監訳者コメント :
この論文で最も重要な知見は、病院環境で最も優勢に検出された Candida 属が Candida albicans ではなく、Candida parapsilosis であった点である。これは近年報告されているカンジダ血流感染症の疫学変化とも一致する結果である。
また、ICU の清掃済みベッドからも Candida DNA が検出されたことは、環境管理の重要性を再認識させる所見である。一方で、本研究で用いられた DNA 解析は生菌の存在を証明するものではなく、環境中に Candida 由来 DNA が残存していることを示すにとどまる。そのため、「環境中への DNA 残存」と「感染リスク」は区別して解釈する必要がある。
