背景
クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)は、医療に関連した下痢の主因の 1 つであり、世界中でかなりの罹患率と死亡率がある。ドイツでは、CDI の発生率が近年減少している。しかし、CDI 患者における院内全死因死亡率や死亡のリスク因子についての多施設による前向きデータは、依然として乏しい。
方法
CDI 患者における院内全死因死亡率を明らかにするために、ドイツの 6 つの大学病院で、2016 年から 2020 年の間に、前向き多施設共同調査研究を実施した。それに加えて、死亡のリスク因子を特定するために、多変量ロジスティック回帰を使用した。
結果
CDI の症例が合計で 3,592 例記録され、そのうちの 46.7%が女性であった。全体の院内全死因死亡率は 11.7%であり、一方、CDI が原因と考えられる死亡率は 0.4%であった(N = 16)。CDI 患者における死亡の独立したリスク因子には、年齢の増加(1 歳当たり 3%、補正オッズ比[aOR]、1.03、95%信頼区間[CI]、1.02 ~ 1.04)、入院が長くなること(1 日当たり 1%、aOR、1.01、95%CI、1.00 ~ 1.01)、患者の複雑性の尺度としての内科への入院(aOR、1.60、95%CI、1.32 ~ 1.94)または血液・腫瘍内科への入院(aOR、2.38、95%CI、1.75 ~ 3.24)、標本採取から ±2日以内のクレアチニン値の上昇(1 mg/dL当たり 6%、aOR、1.06、95%CI、0.99 ~ 1.13)があった。それとは対照的に、一般病棟での診断は(集中治療室または中間ケアと比較して、専門に関係なく)、死亡のリスクの低下と関連していた(aOR、0.34、95%CI、0.28 ~ 0.42)。
結論
我々の研究は、ドイツにおける CDI 患者の院内全死因死亡率について、包括的な評価を提供している。しっかりとしていて使いやすい死亡のリスク因子を特定したことによって、我々の研究結果は、リスク層別化の改善の助けとなる。また、高リスク患者集団に標的を絞った管理戦略に情報を提供できるかもしれない。
監訳者コメント :
加齢、入院期間の延長、腎機能障害、および特定の診療科への入院が、死亡リスクを左右する予後因子であることが示唆された。
