医療スタッフ向けの 6 ステップ手指消毒法を解析する:混合法を用いた有効性、受容性および実行可能性の調査

背景

医療において擦式アルコール製剤(alcohol-based hand rubs:ABHR)を用いる際には 6 ステップ法が推奨されている。しかし、個々のステップの有効性を裏付けるエビデンスは限られており、医療従事者の遵守度は低く、その原因は受容性や実行可能性が低いこと、またはトレーニングが不十分であることであると考えられる。本研究では、医療スタッフ向けの 6 ステップ手指消毒法について、微生物学的有効性、受容性および実行可能性を考慮して有効性を評価した。

方法

混合法デザインを用いた。データは 2017 年から 2020 年まで 2 段階で収集した。第 1 段階では、修正 EN 1500 手順によりラテン方格法を用いて、6 ステップ法と、1 ステップずつ省いた 6 通りの方法とで、細菌数減少および手指表面被覆率を比較した(N = 35)。第 2 段階は、実臨床で 6 ステップ手指消毒法を用いることの受容性および実行可能性に関する横断的オンライン調査であった(N = 78)。

結果

ステップ 4(指の背面)を省くと、完全な 6 ステップ法を用いた場合と比較して、細菌数減少量が有意に減少した(P = 0.001)。ステップ 1(手掌)またはステップ 2(手背)を省いたときには、それ以外のすべての方法と比較して、手指被覆率が有意に低下した(P ≦ 0.001)。第 2 段階の参加者は、6 ステップ法を受容可能で実行可能であると考えており、ステップ 1 が最も受け入れやすく実行しやすいステップであるとした。

結論

ステップ 4 は細菌数減少に最も大きく貢献し、ステップ 1 および 2 は手指被覆率に重要であった。医療従事者は、個々のステップの遵守度を向上させる取り組みとともに、6 ステップ法の使用を継続すべきである。今後の研究において、ステップの順序を変更したときの影響や、石けんと水を用いた手洗い時の各ステップの貢献を検討すべきである。

監訳者コメント

この研究ではステップ 4(指の背面)の手指衛生の工程が細菌量の減少に最も大きく貢献した。ステップ 1(手掌)とステップ 2(手背)は、手指の覆いを確保する上で重要であったという。総じて、一連の手指衛生手順を「ラジオ体操」のように一連の手指衛生の手順をスムーズに執り行うことが重要であろう。
