小児集団における抗菌薬適正使用介入が抗菌薬耐性に及ぼす影響:日本の全国保険請求データおよび抗菌薬耐性サーベイランスデータの分析★

背景
抗菌薬の過剰使用は、抗菌薬耐性(AMR)出現の一因となっている。日本政府は 2018 年に、小児を対象とした抗菌薬適正使用促進プログラムを導入した。しかし、処方パターンや AMR 発生率に対するその影響は、依然として不明瞭である。
目的
本研究の目的は、抗菌薬適正使用促進プログラムの実施が AMR 率に与えた影響を、抗菌薬処方パターンの変化を通して調べることであった。
方法
全国的な請求データベースのデータを用いて2014 年 1 月から 2021 年 7 月までの期間を対象に病床数20 床以上の医療施設から抽出した小児患者データを後向きに解析した。抗菌薬の処方量は 1,000 患者日当たりで計算した。尿中大腸菌の AMR 率は、網羅的な全国 AMR サーベイランスデータベースからのデータを用いて解析した。変化は、分割時系列分析を使って評価した。
結果
介入後、8,292 の医療施設において、広域および狭域抗菌薬の処方傾向はいずれも減少傾向を示したが、第一世代セファロスポリン系薬についてはその傾向は認められなかった。フルオロキノロン系および広域ペニシリン系薬の処方は増加傾向から減少傾向へと転じた一方、第三世代セファロスポリン系薬の処方は減少し続けた。800 を超える医療施設において、尿中大腸菌の広域および狭域抗菌薬に対する AMR のパターンは減少傾向を示した。
結論
抗菌薬適正使用支援促プログラムの導入後、広域および一部の狭域抗菌薬の処方量が減少し、尿中大腸菌の広域・狭域抗菌薬に対する AMR 率が減少した。これらの知見は、小児集団における AMR の抑制に向けた政策主導型の介入の有効性を示唆している。
