背景

手術部位感染症(SSI)は頭蓋修復手術における失敗の原因として最も多くみられるものである。頭蓋形成術に関連する感染症のほとんどは、インプラント抜去を要し、問題のある頭蓋骨欠損が再度生じることとなる。インプラント温存型の頭蓋形成術の感染管理は、小規模患者コホートにおける初期の成功を受けて、臨床的に注目を集めている。われわれは、ケンブリッジ大学病院においてインプラント温存型の頭蓋形成術の感染管理の結果を監査した。

方法

2017 年 11 月から 2021 年 12 月にかけて患者 195 例に実施された頭蓋形成術 206 件を後向きに特定し、頭蓋形成術に関連する SSI の管理および転帰について系統的に評価した。

結果

頭蓋形成術計 201 件(98%)がチタン製インプラントを用いて実施された。患者 67 例(33%)が 1 つ以上の合併症を有していた。SSI は頭蓋形成術 34 件(17%)の実施後に発生した。頭蓋形成術 20 件(10%)で感染症が理由でインプラントが抜去され、頭蓋形成術計 25 件(12%)では合併症が理由でインプラントが抜去された。インプラントを温存した SSI 管理は23 件で試みられ、そのうち 14 件(61%)で成功した。
創滲出の存在はインプラント温存型感染管理の失敗と関連したが(P < 0.003)、創離開、微生物学的培養結果、感染のタイミング、頭蓋形成術実施部位の過去の感染、炎症パラメーター、画像検査における貯留所見、年齢、臨床状態およびシャント治療は失敗と関連しなかった。

結論

患者を適切に選択すれば、インプラント温存型の頭蓋形成術関連感染管理は、実施可能な長期的な結果をもたらすようである。このアプローチは、神経学的状態をモニタリングできる比較的健康な患者で検討できるかもしれない。本研究の結果は、われわれの臨床的意思決定に影響を及ぼしており、われわれは現在、全身状態が良好で、創が治癒しているか、または排膿がないSSI 疑いのすべての患者において頭蓋形成術のインプラントを温存することを目標としている。本研究の結果を確認するためには前向き研究が必要である。

監訳者コメント

頭蓋形成術後の SSI では、感染源の除去を目的としてインプラント抜去が基本とされてきたが、頭蓋欠損の再発や再手術の必要性、神経学的・整容的リスクなどの問題があった。本研究は、インプラントを温存したままの治療が可能かを検証し、全身状態が良好で、創が閉鎖されており排膿がなく、神経学的にフォロー可能といった条件を満たす症例では、「抜去せずに治療可能である」ことを示した点で、従来「原則抜去」とされてきた頭蓋形成術後 SSI の治療戦略に一石を投じるものである。
一方で、本研究は後向き研究で、症例数も限られていることから、結果の一般化には慎重な解釈が必要である。
