病院排水におけるキノロン系薬およびマクロライド系薬に対する耐性の存在率および動態:ネパール、カトマンズ渓谷(Kathmandu Valley)からの洞察★

背景
病院排水は、抗菌薬耐性菌および抗菌薬耐性遺伝子、特にキノロン系薬、フルオロキノロン系薬、およびマクロライド系薬に対する耐性を付与する遺伝子の極めて重要なリザーバとなる。本研究では、ネパール、カトマンズ渓谷(Kathmandu Valley)の病院排水における抗菌薬耐性菌および抗菌薬耐性遺伝子の存在率および季節動態を調べた。
方法
夏季および冬季(2022 年から 2023 年)に、病院 8 施設から未処理の病院排水サンプル 16 個を収集し分析した。物理化学的パラメータ、抗菌薬耐性菌、選択した抗菌薬耐性遺伝子(qnrS、aac(6‘)-Ib–cr、erm(B))および class 1 integron(intI1)の存在率について評価した。得られた結果を、病院のタイプおよび季節により比較した。
結果
有意な季節差は、気温(P = 0.00024)および総浮遊物質(P = 0.042)に認められた。肺炎桿菌(Klebsiella pneumoniae)(31.67%)および大腸菌(Escherichia coli)(28.33%)は、最も多く分離された抗菌薬耐性菌であった。肺炎桿菌では、シプロフロキサシン(97.74%)およびレボフロキサシン(89.47%)に対する耐性率が非常に高いことが認められた。対象とした遺伝子のうち、遺伝子 aac(6‘)-Ib–cr は最も存在率が高く(55.67%)、次いで intI1(50%)、qnrS(25%)および erm(B)(11.67%)の順であった。遺伝子 erm(B) は中規模病院において存在率が有意に高かった(P = 0.001)。抗菌薬耐性遺伝子または可動遺伝因子について有意な季節変動は認められなかった(P > 0.05)。統計学的に有意ではなかったが、qnrS の存在率と pH(ρ = 0.912)およびアンモニア(ρ = 0.812)との間に強い相関が認められ、このことから耐性の拡散に環境が影響する可能性が示唆された。
結論
病院排水は抗菌薬耐性の重要なリザーバであり、このことは耐性の拡散を低減するため、排水処理および抗菌薬適正使用支援の改善の必要性を浮き彫りにしている。
監訳者コメント :
ネパールの病院 8 施設におけるqnrS・aac(6′)-Ib-cr・erm(B)・intI1の同時検出を初めて包括的に示した報告である。特に K. pneumoniae が複数の耐性遺伝子とクラス 1 インテグロンを同時保有し、水平伝播の主要な担体となっている可能性を示したことは公衆衛生上重要な知見である。また、erm(B)の中規模病院への集中という病院規模別の偏在パターンは、施設特性と耐性遺伝子分布の関連を示した先駆的観察である。さらに環境因子(pH・アンモニア)と qnrS 拡散の関連示唆も新たな仮説として提示された。70%超の医療施設が排水処理設備を持たないというネパールのインフラ問題が依然として根本的課題として残っており、処理設備整備・AMS プログラム導入・AMR サーベイランス体制の構築が喫緊の課題である。
