背景

抗菌薬投与前の血液培養検査は、感染症の診断および治療にとって不可欠である。日本において、抗菌薬適正使用支援チームは広域抗菌薬および抗メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)抗菌薬のモニタリングを行っており、これらの抗菌薬は多くの病院で通知システムによって管理されている。しかし、抗菌薬適正使用支援チームの薬剤師による、血液培養検査の実施率改善のための介入に関する報告はほとんどない。本研究は、抗菌薬適正使用支援チームの薬剤師による血液培養検査を促す通知の有効性を評価することを目的とした。

方法

指定(制限的あるいは許可制)抗菌薬(静注広域抗菌薬および抗 MRSA 抗菌薬)を処方された全入院患者のモニタリングを行い、その適応を評価した。抗菌薬適正使用支援チームの薬剤師は、2023 年 4 月に血液培養検査が実施されたかの確認を開始した。血液培養検査が実施されていなかった場合、薬剤師は患者の電子カルテに、2 セットの血液培養を行うよう促すプロンプトを手動で記録した。処方された指定抗菌薬と血液培養検査セットについて、ベースライン期間(2022 年 4 月 から 2023 年 3月)と介入期間(2023 年 4 月から 2024 年 3 月)の間で、Mann-Whitney U 検定を用いて遵守の変化を比較した。

結果

血液培養検査の実施率は、指定抗菌薬投与の前には汚染率の上昇なしに有意に高く(44.6% 対 51.9%、P < 0.05)、二次効果として、抗菌薬適正使用支援チームの薬剤師からの通知後には指定抗菌薬の処方が減少した(15.6 対 13.2、P < 0.05)。

結論

抗菌薬適正使用支援チームの薬剤師による、医師に対して血液培養検査を促す通知は、血液培養検査の実施率を改善し、指定抗菌薬の処方を減少させる可能性がある。

監訳者コメント

抗菌薬適正使用のために、広域あるいは抗 MRSA 抗菌薬を使用前に薬剤師が血液培養を実施するように促す「コメント」を電子カルテ上に記載するという軽度の介入においても医師の行動変容が見られ、培養率や適正使用に改善がみられたことは意義深い。さらに薬剤師の AST での役割を拡げ、diagnostic stewardship が antimicrobial stewardship に影響していることも明らかとなった。患者背景や臨床的なアウトカムがどのようになっているのかがさらに知りたいところであり、さらなる臨床研究の継続により、これらが明らかにされることを期待したい。
