背景

紛争や大規模災害は、多国間での医療の提供を危うくし、カルバペネム耐性菌(CRO)が拡散するリスクとなる。英国における渡航関連 CRO のリスクや検出能力の実態は明らかではない。

方法

英国の急性期 NHS 病院・地域・委員会の 108 施設の微生物・感染防止管理(IPC)担当者を対象に 10 問の質問から成る調査票を送付し、渡航関連 CRO に関する最近の経験および IPC の実践と、広域抗菌薬検査への取り組み方を調査した。さらに 2022 年 3 月から 2024 年 4 月に主要外傷ネットワーク施設で検出されたカルバペネマーゼ産生菌(CPO)の分子データを世界保健機関(WHO)地域別に一元配置分散分析でに比較した。

結果

回答は 73 施設で IPC への取り組みには非常にばらつきがあり国家スクリーニングガイドラインの修正を要した施設が 26.0%(73 施設中 19 施設)であった。73 施設中 24 施設(32.9%)が、主要な紛争地域渡航関連 CRO を検出した主要外傷ネットワーク施設 12 施設対象の調査で、1,290 例中 297 例(23.0%)が異なる計 52 か国の渡航歴を有する CPO 保有者であることが判明した。検出法は 297 例中 227 例(76.4%)がスクリーニング、279(93.9%)がは腸内細菌目細菌であった。渡航先は297例中112例(37.7%)が欧州でカルバペネマーゼの多様性がその他の地域より大きかった(P < 0.001)。

解釈

かなりの範囲の英国の施設が、現在主要な紛争地域への渡航関連CRO を検出している。入院時に CPO のリスクを層別化するには、初診時に、渡航歴についてより教育的に取り組む必要がある。これらのデータは、渡航関連感染症のリスクを評価するために、効果的に旅行歴を聴取することを組み入れるのに成功したプロジェクトと並行して、前向きに収集すべきである。これによって、渡航先での行動や流行をより明確に理解することが可能となり、リスクを正確に記述できるようになり、IPC を効果的なものにするための情報が提供できるようになる。
