Hand hygiene compliance and glove use in intensive care units: a one-year observational study in Hungary

背景

医療従事者の適切な手指衛生は、医療関連感染（HAI）の予防において極めて重要な因子である。世界保健機関（WHO）は、手指衛生の遵守の向上を、感染予防における優先事項の 1 つとして特定している。

目的

手指衛生の遵守を向上させ、教育、継続的な監視、フィードバックの有効性を評価し、手指衛生の遵守と HAI の関係を明らかにすること。

方法

手指衛生の遵守を、ハンガリーの Nyíregyháza にある Szabolcs-Szatmár Bereg County HospitalとUniversity Teaching Hospital の集中治療室で、1 年間に亘り、WHOの直接観察法を使って監視し、7,473 件の事例を記録した。HAI の発生率（治療された患者 100 例当たり）は、1,002 例の患者の医療記録の後向き解析によって評価した。

結果

3 つの治療科は、異なる遵守レベルを示した。内科集中治療科 II は、他の 2 つの科と比べて、有意に異なる手指衛生の遵守と HAI 発生率を示した。集中治療室全体と外科内では、手指衛生の遵守と HAI 発生率の間に有意な相関が見出された。手袋の正当化されない使用と手指衛生の不遵守の間に、測定可能な相関があった。

結論

教育、継続的な監視、フィードバックは、手指衛生の遵守の向上に重要な役割を果たすことで、HAI の減少に貢献している。しかし、影響を与えるそれ以外の因子をいくつか考慮しなければならない。遵守と HAI の関係が確認されたのは、1 つの科のみである。
