背景

手術室の空気の質は、手術部位感染症(SSI)を予防するために重要である。HVACシステムは、高い空気の質を保証し、微生物汚染を最小限に抑えるものであるべきである。近年、SSI の予防における下降流超清浄空気(UCA)システムの効果が議論されている。

目的

イタリアのマルケ地方にある病院 12 施設から得たデータ(2018 年から 2019 年)を用いて、手術室の HVAC システムと SSI 発生率との相関を分析した。

方法

データを、899 例の外科手術後の SSI 発生率と関連付けた。ロジスティック回帰分析により、UCA システムは SSI リスクの低下に関連することが示された(オッズ比 0.37、95%信頼区間[CI]0.15 ~ 0.92)。

結果

38 例の SSI が認められ、全発生率は 4.2%(95%CI 3.0 ~ 5.8)であった。このうち、29例は非 UCA システム下で発生し(5.17%、95%CI 3.5 ~ 7.4)、9 例は UCA 環境で発生していた(2.66%、95%CI 1.2 ~ 5.0)。各手術室における検証試験から、ISO 5 クラスの基準に適合する一方向流型換気を備えた UCA HVAC システムに的を絞って、技術データを収集した。

結論

以上のデータから、UCA は、非 UCA 換気システムと比較して、微生物空気汚染を減少させ、それによって空気の質を改善し、SSI のリスクを低下させる効果を有する可能性が示唆される。
