A systematic review of the cost impact of sepsis care bundles

背景／目的

敗血症への臨床的対応のばらつきを最小限にし、健康転帰を改善し、関連する費用を改善するために、エビデンスに基づく敗血症バンドルを使うことは、特に高度急性期医療では、現在の文献で十分に支持されている。しかし、亜急性期の高齢者の居宅介護施設や、より幅広い急性期ケアの環境で実施された敗血症バンドルを評価した研究は限られている。このシステマティックレビューの目的は、急性期および亜急性期の医療機関と、高齢者の居宅介護施設における敗血症ケアバンドルの使用／実施に関連した財政的な転帰を報告している研究を評価することであった。

方法

Cochrane データベースと、CINAHL Complete および MEDLINE Complete（EBSCOHost プラットフォームを介して）の電子検索を、2000 年 1 月から 2025 年 7 月にかけて実施した。研究は、2 名の独立した研究者がスクリーニングをし、評価した。

結果

12 件の研究が組み入れの候補として特定され、これらのうち、58.33％（n ＝ 7）が確立している経済評価の方法論を使用していて、5 件の研究（41.67％）が敗血症バンドルを実施することの費用面での影響の概略を述べていた。結果は、報告されている費用の転帰や敗血症の定義が均一ではないことをはっきりと示していた。また、患者のレベルでのデータや評価が限られていた。研究結果が均一ではなかったにもかかわらず、このレビューの結果は、臨床的および経済的有効性が急性期ケアの現場での敗血症バンドルの実施と関連していることを示している。敗血症バンドルの実施は、66.7％の研究で、病院および／または患者の費用の減少と関連していた。

結論

敗血症バンドルの経済的な影響を、亜急性期ケア施設や高齢者の居宅介護施設のように、急性期ケアの環境を超えて、小児科の集団や成人の集団を対象に評価するには、更に研究が必要である。それに加えて、費用・影響の評価方法の管轄区域を超えた標準化により大きな関心を向けることが、成績の基準に照らした評価と、転帰のメタアナリシスを促進するには必要である。
