抗微生物薬の選択を導く臨床意思決定支援システム:2019 年から 2023 年までの記述的レビュー

医療提供を改善するために、臨床意思決定支援システム(CDSS)の開発が進んでいる。このテーマに関する最近のレビューを更新するために、2019 年から 2023 年までの Medline の検索により特定された、抗微生物薬の処方を専門とするすべての CDSS を説明することを目的として、記述的レビューを実施した。抗菌薬、抗ウイルス薬、抗真菌薬、抗寄生虫薬を含む抗微生物薬の選択を専門とする CDSS を記述したすべての原著論文を適格とした。CDSS の目的およびそれらが及ぼしうる臨床的影響、さらには CDSS を評価するための研究の特性(研究の種類、対象集団、設定、目的、主な結果)を抽出した。組み入れた研究 28件のうち、ほとんど(24 件)は抗菌薬専門であり、15 件が電子カルテに組み込まれていた。低中所得国で開発された CDSS は 1 つのみであった。組み入れた CDSS の主な目的は、抗微生物薬の選択において臨床医を支援することであった。10 件の研究で 1,000 例を超える患者または処方が組み入れられ、そのほとんどは病院で実施されていた。6 件の研究でのみ、抗菌薬の使用またはガイドラインの遵守に対する CDSS の影響はないと報告された。抗微生物薬処方を専門とする CDSS は活発になりつつある分野であり、その解答は抗微生物薬の使用を最適化する可能性をもたらす。CDSS の開発が急務であるが、CDSS の臨床評価が導入前の重要な要件であると考えられる。教育と訓練を通じて認識を向上させながら、CDSS の移転可能性と高い採用率を実現するためには、特に低中所得国において、特別な努力が必要である。
監訳者コメント:
抗微生物薬の処方に臨床意思決定支援システム(CDSS)の活用が考えられるが、耐性菌による感染症のリスクを十分に考慮して検討すべきであろう。
