Barriers and facilitators to the use of personal protective equipment in long-term care: a qualitative study

背景

長期ケア（LTC）施設の入所者は、侵襲性感染症に対し脆弱である。個人防護具（PPE）の適切な使用と訓練は、入所者と医療従事者を守るのに重要である。PPE の使用の障壁と促進要因について、LTC施設での研究は限られている。

目的

アルバータ州カルガリーの LTC 施設における PPE の導入および適切な使用の障壁と促進要因についての医療従事者の見方の特徴を明らかにすること。

方法

2022 年の 4 月から 10 月に、医療従事者の半構造化インタビューを実施した。インタビュー記録を演繹的に分析し、理論的領域フレームワークからテーマを特定した。

結果

7 名の医療従事者にインタビューを行った。障壁と促進要因は、6 つの包括的なテーマにまとめられた。すなわち、PPE の利用可能性と品質、PPE の使用方法の知識、家族の義務、利便性と快適性、職業上の義務感、社会的な影響とアイデンティティである。それ以外の要因として、例えば、人員不足と研修会を増やす必要性がはっきりと示された。医療従事者によって特定された PPE の使用を改善するための戦略は、PPE チャンピオンの活用、定期的な監査、建設的なフィードバック、等であった。

結論

LTC 施設の医療従事者が PPE の使用に関する独特な障壁と促進因子を特定することで、この状況において PPE の使用を改善するための標的を絞った介入が促進されるだろう。
