VanB transposon analysis detects horizontal gene transfer in vancomycin-resistant Enterococcus faecium: description of two outbreaks

背景

バンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）（VRE）のアウトブレイクは、世界中の病院で多くみられる。アウトブレイク株の同定には全ゲノム MLST がしばしば用いられるが、ゲノム変異が限られているため、VRE タイピングは依然として困難な場合がある。

目的

全ゲノム MLST と並行してバンコマイシン耐性遺伝子の配列解析を行う方法を開発し、この新しい方法を 2 件の並行 VRE アウトブレイクのリアルタイム調査に適用する。

方法

vanB 耐性遺伝子を含むトランスポゾンの配列比較のためのバイオインフォマティクスパイプラインを開発した。このパイプラインを 全ゲノム MLST に加えて使用し、進行中の 2 件の別々の VRE アウトブレイクを調査した。15 の異なるサンプルから得られた 5 個の別々のコロニーと、10 のバンコマイシン感受性分離株の配列解析も行った。

結果

2 件のアウトブレイク中に収集された 46 株のうち、全ゲノム MLST クラスタリングに基づいて、26 株と 9 株が 2 件のアウトブレイクの一部であることが確認された。6 株は、vanB トランスポゾンは同一であるが、全ゲノム MLST クラスターが異なることが確認され、水平遺伝子伝播が示唆された。このアウトブレイク拡大の可能性は、トランスポゾン解析を行わなければ見逃されていた可能性がある。同じサンプルの異なるコロニー内では、vanB トランスポゾン配列や 全ゲノム MLST プロファイルのばらつきを示す根拠は認められなかった。血液培養でバンコマイシン感受性 E. faecium が 1 株同定され、その 全ゲノム ST はアウトブレイク株のうちの 1 つと類似していた。

結論

バンコマイシン耐性遺伝子を含むトランスポゾンのリアルタイム解析は、vanB-VRE アウトブレイクの解析に追加情報を提供する。これにより、従来の方法では検出できない可能性がある水平遺伝子伝播の可能性を検出できる。トランスポゾン解析は、vanB-VRE アウトブレイク時の全ゲノム配列解析に追加できる有益な解析法である。
