Effects of reminders from Sanibit™ electronic hand hygiene system and incentives on hand hygiene compliance: a prospective, four-phased, observational study

背景

自動手指衛生モニタリングシステムによる個別化されたリマインダーとインセンティブは、手指衛生の有意な改善に寄与する可能性がある。

目的

3 次ケア病院における 4 期にわたる前向き試験で、集中治療室における Sanibit™ 自動手指衛生モニタリングシステムの実装についてと、これらの介入が手指衛生遵守に及ぼす影響について包括的に評価すること。

方法

Sanibit モニタリングシステムを、10 床の脳疾患集中治療室に設置した。第 1 期はリマインダーなしで現金によるインセンティブ、第 2a 期は光のみによるリマインダーと現金によるインセンティブ、第 2b 期は光 ＋ 振動によるリマインダーと現金によるインセンティブ、第 3 期はリマインダーなしでポイントシステムによるインセンティブ、第 4 期は光 ＋ 振動によるリマインダーとポイントシステムによるインセンティブを用いた。

結果

Sanibit による検出の正確度は、直接観察と比較して 85.1％であった。光のみによるリマインダーと現金によるインセンティブを用いた第 2a 期では、手指衛生の機会が週半ばに最も多くなった。光 ＋ 振動によるリマインダーとポイントシステムによるインセンティブを用いた第 4 期では、手指衛生の総遵守率、完全遵守率、および不完全遵守率が最も高くなり、一方、光のみによるリマインダーと現金によるインセンティブを用いた第 2a 期では、手指衛生の機会が最も多くなった。入退室時間が短い場合は、手指衛生遵守率は退室時の方が入室時よりも有意に低くなったが、入退室時間が長い場合は、その逆になった。

結論

手指衛生の機会、手指衛生遵守率、および患者接触時間は、種々のリマインダー／インセンティブ、個々の医療従事者、曜日、および 1 日の時間帯の間で有意な変動が認められた。光 ＋ 振動によるリマインダーとポイントシステムによるインセンティブを用いた場合に、最も高い手指衛生遵守率が達成された。
