歯科診療所における環境表面および個人防護具の汚染と制御:多施設共同研究★

背景
歯科処置は、エアロゾル、血液、唾液の飛散の産生と関連している。しかし、歯科処置中の血液による表面の汚染の程度は、大部分が依然として不明である。
目的
歯科診療所における環境表面および個人防護具(PPE)の汚染、およびその後の消毒処置の効果について評価すること。
方法
この研究は、上海の 5 つの歯科診療所で実施した。歯科用椅子周囲の環境表面、手袋、顔面防護具の汚染を、潜血の検出およびアデノシン三リン酸(ATP)生物学的蛍光分析を使って定量した。ATP 検査の結果は、相対発光量(RLU)で示す。標準化した清掃と消毒のプロトコールを実行に移した後、さらに分析を実施した。
結果
手袋に潜血が存在したのは、抜歯中が最も頻度が高かったが(67.65%)、RLU の最高値は、かぶせ物や歯列矯正器についての処置で観察された(中央値、3,214、四分位範囲[IQR]18.00 ~ 5,371.00)。顔面防護具の潜血陽性率は、24.00%(RLU の中央値、59、IQR、14.75 ~ 206.75)であった。潜血による汚染が最もひどかった環境表面は、歯科用椅子の流し(87.00%)、ハンドピースのハンドル(48.00%)、唾液排出器のハンドル(36.00%)であった。消毒処置によって、すべての場所で汚染のレベルが有意に低下し、両方の測定方法(潜血の検出と ATP検査)において、歯科用椅子の流し、唾液排出器のハンドル、ハンドピースのハンドルの汚染レベルの減少が示された。
結論
この研究は、歯科診療所において、汚染のリスクが高い場所と処置を特定した。消毒は、血液による表面の汚染に対して有効である。
監訳者コメント :
歯科処置における血液体液曝露防止は医療従事者を感染から守る上で重要であり、診療環境の清掃と消毒とともに、適切な PPE については調査結果が証明しているように着用は必須である。本論文では主に 0.2%の 4 級アンモニウム塩と 1%過酸化水素のワイプが使用されており、適切に汚染部位を確実に消毒すると共にこれらの血液体液を拭いとることが重要である。
