Personal protective equipment stewardship across 112 medical facilities: a preliminary survey in Japan

背景

個人防護具（PPE）の管理は、医療従事者の保護により医療関連感染症を予防する上で極めて重要である。

目的

日本の医療施設における PPE の選択プロセスおよび PPE 管理 の現状を包括的に明らかにすること、ならびに日本における PPE 管理 の質を高める上で貢献するであろうエビデンスを収集すること。

方法

2024 年 2 月から 3 月の間に、日本の医療施設 200 施設で PPE 管理に関する質問票調査を実施した。

結果

回答率は 56.0％（200 件中 112 件）であった。回答した施設の 99.1％がPPE 使用に関する文書化されたプロトコールを有し、包括的な訓練（実践的スキルを含む）を実施しており、3.8％のみがこの訓練を外部委託スタッフにまで拡大実施していた。58.0％の施設で複数サイズのサージカルマスクが利用可能となっており、58.9％の施設はN95マスクのフィットテストを実施する設備を備えていた。さらに、58.9％の施設が PPE 管理のモニタリングおよび評価を、環境ラウンドおよび手指衛生評価と併せて実施していた。

結論

サンプルサイズが限られていたものの、本研究は日本における PPE 使用に関する初の大規模調査である。今回の結果は、今後調査を行うための基礎と、PPE に関する最適なプロトコールを確立するためのエビデンスを提供している。さらにこの結果は、医療関連感染症の制御を前進させる上で貢献する可能性がある。
