Factors associated with core competencies of infection prevention and control practitioners in 511 hospitals: a large cross-sectional survey in Guizhou, south-west China

目的

中国南西部貴州省の複数の病院における感染予防管理担当者（IPCP）のコアコンピテンシーのレベルに影響する決定因子を調べること。

方法

本研究では、2022 年 4 月から 6 月にかけてIPCP を対象に横断調査を実施した。4 つのコア領域、11 のサブ領域および 47 の評価項目から成る、IPCP を対象としたコンピテンシー自己評価尺度を用いた。

結果

病院 511 施設の IPCP 1,083 名における自己評価コンピテンシースコアの平均 ± 標準偏差（SD）は、4.891 ± 1.204 であった。自己評価スコアの最低平均 ± SD は、4 つのコア領域では専門的職業開発能力の 4.494 ± 1.291 であり、11 のサブ領域では包括的知識の 3.748 ± 1.578 であった。異なる病院におけるコアコンピテンシーについては、中国伝統医学病院のスコア（5.306 ± 1.071）で最も高かった。線形回帰分析から、自己評価によるコンピテンシーに影響を及ぼす独立した因子は、年齢（B ＝ 0.487、95％信頼区間［CI］0.189 ～ 0.785）、感染予防管理（IPC）の実践経験年数（B ＝ 0.216、95％CI 0.050 ～ 0.382）、役職（B ＝ －0.395、95％CI －0.693 ～ －0.097）、月収（B ＝－0.296、95％CI －0.484 ～ －0.107）、第一線のエピデミック対応の経験（B ＝ 0.236、95％CI 0.098 ～ 0.382）、オフィスソフトウェアアプリケーションの技能（B ＝ ―0.747、95％CI －0.898 ～ －0.596）、および病院における IPC の文化的雰囲気（B ＝ －0.406、95％CI －0.799 ～ －0.013）であった。

結論

中国の IPCP におけるコアコンピテンシーは強化する必要がある。異なる病院の IPCP のコアコンピテンシーにおける違いが明らかにされ、コンピテンシーに基づくトレーニングプログラムのさらなる開発を支持する強いエビデンスが得られた。
