病院内のアスペルギルス症のリスクを低減するための現行の国内および国際ガイダンスの調査★

アスペルギルス（Aspergillus）属は、重症免疫不全の宿主や慢性胸部疾患を有する宿主における疾患に最も高頻度に関連する。リスクがある患者の範囲は拡大しており、集中治療（重症ウイルス性肺炎を含む）、外傷、熱傷および大手術などがあげられる。曝露または保菌がアスペルギルス関連疾患の前提条件であることから、医療機関において推奨される予防策の国際的レビューを行うことにした。本国際的レビューでは、24 カ国の 75 件の文書を組み入れ、侵襲性アスペルギルス症（IA）予防のための臨床、感染予防・制御および建物関連のガイダンスに分類した。IA の潜伏期間と、救命救急室や手術室などの防護環境における空中アスペルギルスの異なる許容濃度を概説する。防止、予防、回避、予防策およびモニタリング（環境および臨床）のすべての側面に対応する文書は少なかった。複数のリスクを特定および最小化し、適切な予防策を確実に実施するためには、集学的なアプローチが必要である。建物関連のガイダンスのほとんどは、建設および病院内部のリフォームに対応するものであるが、本レビューでは、医療環境（換気装置を含む）の良好な管理の重要性や環境モニタリングの不確実性についても考察する。本レビューでは、救命救急環境を含む防護的な患者環境に推奨される基準の違いを明らかにする。防護環境に必要な大規模な設備投資は、最もリスクの高い患者群に限定される場合が多い。包括的ガイダンスとなる単一の文書はなく、多数の国がガイダンスを提供していない。易感染性の入院患者の発症期間における侵襲性アスペルギルス症の医療関連感染を減少させるためには、リスクがある患者に関する認識を高め、綿密なリスク評価を行い、通常の病院環境を注意深く維持する必要がある。

この論文は、各国で「易感染性の入院患者の発症期間における侵襲性アスペルギルス症の医療関連感染を減少させるためのガイダンス」がどのように提供されているのか国際的に比較・調査したものである。その結果、「多数の国がガイダンスを提供していない」という現実が明らかとなった。
