早期の抗ウイルス薬使用および酸素補充が二次細菌感染のリスクを低下させる：多施設共同コホート内症例対照研究★

背景
本研究の目的は、新型コロナウイルス感染症（COVID-19）期間中の二次細菌感染に対する宿主感受性を決定する治療戦略について検討することであった。
方法
本コホート内症例対照研究は、2022 年 12 月 1 日から 2023 年 3 月 1 日に中国の 3 つの総合病院で実施した。COVID-19 確定患者合計 456 例を、年齢、性別、疾患重症度、年齢調整 Charlson 併存疾患指数（aCCI）に基づき、傾向スコアマッチング（PSM）を用いて 1：2 でマッチさせて組み入れた（症例 152 例、対照 304 例）。二次細菌感染と、対症療法、抗ウイルスおよび抗菌療法を含む治療戦略との関連を、主要評価項目とした。
結果
条件付きロジスティック回帰分析により、カテゴリー変数のうち、抗菌薬、抗ウイルス薬、ヒト免疫グロブリン静注、グルココルチコイドまたは抗凝固薬の使用に、COVID-19 患者における二次細菌感染リスクとの関連は認められなかった。酸素補充は低流量（オッズ比［OR］0.18、P ＜ 0.001）または高流量（OR 0.06、P ＜ 0.001）のいずれも、二次細菌感染に対する有意な防御との関連が認められたが、人工呼吸器による酸素療法にはそのような関連は認められなかった。一方、胃管からの栄養（OR 10.97、P ＜ 0.001）または経静脈栄養（OR 3.97、P ＝ 0.002）には、二次細菌感染リスクの有意な上昇との関連が認められた。連続変数を用いてデータを解析した場合も、同様のデータが得られた。さらに、早期（症状発現後 5 日未満）の抗ウイルス薬使用に二次細菌感染に対する防御との関連が認められたが（OR 0.09、P ＜ 0.001）、後期の抗ウイルス薬使用にはそのような関連は認められなかった。
結論
人工呼吸器非装着での酸素補充および早期の抗ウイルス薬使用に、二次細菌感染の発生率低下との関連が認められた一方、経静脈栄養または経管栄養に、二次細菌感染の発生率上昇との関連が認められた。
