背景
スペインの Zero Surgical Infection Project(ZSIP)は、手術部位感染症(SSI)を予防することを目的とした全国規模のプログラムで、保健省 が資金を提供した。このプログラムには 5 つの重要な予防対策のバンドルの適用が含まれている。すなわち、周術期抗菌薬予防投与、皮膚の消毒、除毛、正常体温、正常血糖である。
目的
2017 年から 2021 年のスペインの公立病院の一団で、スペインにおける外科医療の標準的な条件下で適用された 5 つの重要な予防対策の有効性を評価すること。
方法
ZSIP Registry(National Database Network)からの 33,240 例の患者を対象としたこの前向きコホート研究には、選ばれた外科的介入におけるSSI のデータ、コンプライアンスのデータ、その他のリスク因子についてのサーベイランスが含まれていた。5 つの重要な予防対策のバンドルのコンプライアンスは、特異的なチェックリストで確認した。介入群は、5 つの重要な予防対策のバンドルを遵守した患者、比較群は、5 つの重要な予防対策のバンドルを遵守しなかった患者とした。補正オッズ比(OR)と 予防割合 はロジスティック回帰を使って計算した。
結果
全体での SSI の発生率は、遵守群の患者が 4.77%、非遵守群の患者が 6.90% であった。5 つの重要な予防対策のバンドルの全体でのコンプライアンスは 35.9%、遵守率は周術期抗菌薬予防投与が最も高く(87.2%)、続いて、皮膚の消毒(82.2%)、除毛(78.7%)であった。5 つの重要な予防対策のバンドルのコンプライアンスは、予防割合が 32%、補正 OR が 0.68(95% 信頼区間0.60 ~ 0.76)であった。
結論
スペインで 2017 年から 2021 年に実施された ZSIP は、SSI に対して有意な有効性を示した。
