日本の新生児集中治療室におけるパルスキセノン紫外線消毒の臨床的・微生物学的有効性★★

背景
メチシリン耐性黄色ブドウ球菌（Meticillin-resistant Staphylococcus aureus：MRSA）は、新生児集中治療室（NICU）における重大な医療関連感染症の原因菌であることが多い。成人 ICU において、アルコール製剤による手指消毒および最終清掃に加えた環境表面のパルスキセノン紫外線消毒により、MRSA 獲得率が低減することが確認されている。
目的
NICU におけるパルスキセノン紫外線使用が、MRSA 伝播の低減に及ぼす影響を調査すること。
方法
2021 年 1 月から 2022 年 12 月までのすべての患者を対象に、新たに確認された MRSA 症例の罹患率を記録した。最終的な用手清掃に加えて紫外線照射を使用した。ベースライン期間では、MRSA 患者の使用後の領域に照射を実施し、介入期間では、各患者の使用後に照射を実施した。
結果
試験期間を通して、手指衛生遵守率に変化はみられなかったが、検出された MRSA の 1,000 患者日あたりの全症例数は有意に低減した。
結論
NICU において、手指衛生実施と従来の環境整備に加えた環境表面の紫外線照射は、MRSA 感染症の低減に有効であろう。
監訳者コメント：
日本の岐阜県総合医療センターの NICU からの、パルスキセノン紫外線を用いての医療環境の消毒の効果の報告である。1,000 患者日あたりの検出された MRSA の全症例数は、有意に低減したという。日本では NICU における MRSA の検出が問題になっており、本報告の成果が着目される。
