体外式膜型人工肺を使用している小児患者における院内感染:システマティックレビューおよびメタアナリシス★★

院内感染は、体外式膜型人工肺(ECMO)治療の重大な合併症となっており、予後不良と密接に結び付いている。研究では、小児患者は ECMO が有益となる可能性がより高いことが示されている。しかし、小児患者における院内感染は包括的に解析されていない。本レビューでは、ECMO を使用している小児患者における院内感染の発生率、タイミング、部位、主要病原体、関連する抗菌薬の使用およびリスク因子、死亡率への影響を系統的に解析した。7 つのデータベースを検索し、適格な論文を選択した。Stata 15.0 を用いて統合効果および 95%信頼区間を算出し、その他のデータには記述統計を用いた。合計 31 報の論文を組み入れた。院内感染発生率は 0.19(95%信頼区間[CI] 0.17 ~ 0.22)であった。呼吸器感染が最もよくみられる感染型として特定された。ブドウ球菌属菌(Staphylococcus)が主要病原菌であった。抗菌薬使用はすべての施設で広く行われていた。長期にわたる ECMO 補助に、患児における院内感染の発生率上昇との関連が認められた(オッズ比[OR]= 1.09、95%CI 1.06 ~ 1.13)。院内感染に死亡率上昇との関連は認められなかった(OR = 1.44、95%CI 0.98 ~ 2.11)。結論として、院内感染は小児 ECMO 装着患者においてよくみられ、様々な因子による影響を受けていた。しかし、院内感染は死亡リスクを上昇させなかった。

監訳者コメント

ECMO による治療をおこなっていた小児患者例を、汎用統計ソフト「Stata」の Version 15.0を用いて解析を行った論文である。これら ECMO による治療を行っていた患児では、ブドウ球菌属菌(Staphylococcus)による呼吸器感染症が見いだされたが。長期にわたる ECMO で治療を行っていた患児でも院内感染の発生率上昇との関連が認められたが、院内感染は死亡リスクを上昇させなかった。小児 ECMO 療法の治療効果とそれに伴う合併感染とのせめぎ合いが問題である。
