背景
バンコマイシン耐性腸球菌属(VRE)の地域的流行状況にない医療環境における VRE アウトブレイク時について、便または肛門スワブによる VRE に対する積極的サーベイランス培養(ASC)の標的集団は、十分に明らかにされていない。
目的
バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium)アウトブレイク時について VRE 検出の累積発生率を評価して、ASC の適切な標的集団を明らかにすること。
方法
症例には、2022 年 2 月から 2023 年 1 月に静岡県立総合病院で初回 VRE 陽性サンプルが得られた入院患者を対象とした。同期間中に便および肛門サンプルを用いて、高リスクを有する可能性のある患者に対する入院時スクリーニング、全入院患者に対する2週間毎のスクリーニング、HCUにおける入棟時および退棟時スクリーニング、ならびに各病棟における接触者のスクリーニングを実施した。スクリーニングを受けた被験者について、患者特性または曝露可能性に従って、VRE 検出の累積発生率を算出した。
結果
同定された症例 60 例中、55 例(92%)は ASC によるものであった。VRE 検出の累積発生率は、接触者(6.4%、234 例中 15 例)のほうが、他のスクリーニング法で同定された被験者(0.5%、8,565 例中 40 例)よりも高かった。入院時スクリーニングにより同定された患者では、VRE アウトブレイクが報告された地域の病院に入院歴のある患者で最も累積発生率が高く(6.6%、78 例中 5 例)、次いでトイレ介助を必要とする患者であった(3.7%、161 例中 6 例)。バンドルを用いたアプローチ、すなわち病院のICT(感染制御チーム)、地域の保健所、ならびに地域および国の感染制御専門家によるASCおよび迅速接触予防策を含んだアプローチは、7 か月でこのアウトブレイクを終息させる助けとなった。
結論
接触者、VRE アウトブレイクが発生した地域における入院歴を有する患者、ならびにトイレ介助を必要とする患者は、VRE 検出の対象となる高リスク集団と考えられ、ASC の対象候補である。
監訳者コメント:
静岡県からの報告。VRE の検出は日本では少ないが、地域での流行が散発されるようになってきており、薬剤耐性(AMR)対策アクションプラン(2023 – 2027)においても感染症罹患数を 80 人以下にすることが成果指標となっている。新規性は少ないが、日本からの報告であり、確認の意味を含めて一読されたい。
