Epidemiologic characteristics and risk factors of Clostridioides difficile infection in patients with active tuberculosis in the Republic of Korea: a nationwide population-based study

背景

抗結核薬とクロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)との関連は、依然として不明である。本研究の目的は、結核患者における CDI の疫学的特徴とリスク因子について検討することである。

方法

韓国において、2018 年 1 月から 2022 年 12 月に今回の全国集団ベースコホート研究を実施した。National Health Insurance Service-National Health Information Database からデータを抽出した。年齢と性別に基づく 1:4 の貪欲マッチング法を用いて、多変量ロジスティック回帰分析により結核患者の CDI リスク因子を特定した。

結果

研究期間に、抗結核薬を処方された結核患者 131,950 例中 2,901 例が CDI を発症した。韓国における結核患者の CDI 発生率は毎年増加しており、2018 年に 1,000 例あたり 12.31 例、2022 年には 1,000 例あたり 33.51 例であった。CDI に対するもっとも一般的な第一選択薬は経口メトロニダゾール(81.94%)であった。結核に CDI を合併した患者の院内死亡率は 9.9%であったのに対して、結核単独感染の患者では 6.9%であった(P < 0.0001)。多変量ロジスティック回帰分析により、結核患者における CDI 発症の有意なリスク因子として、集中治療室入院、Charlson Comorbidity Index ≧ 3、抗菌薬曝露、標準レジメン、多剤耐性結核、肺外結核が確認された。

結論

結核患者の CDI 発症は稀であるが、発症により死亡率が有意に増加する。結核治療患者では、CDI 発症について慎重に監視すべきである。結核治療期間における CDI の予防ならびに制御のための有効な介入を特定するために、さらなる臨床研究が必要である。
