A nationwide mixed-methods study of gaps and barriers to implementation of antimicrobial stewardship programmes in hospitals in Indonesia

背景

低・中所得国における抗菌薬適正使用支援プログラム(ASP)の実施上の阻害因子について理解する喫緊の必要がある。

方法

インドネシア全州の公立および私立病院において混合手法による研究を実施した(2023 年 3 月から 12 月)。スコア判定法による自己評価質問票を用い、またマルチレベル順序回帰を用いて病院および地域レベルの特性との関連を評価した。病院の利害関係者とのフォーカス・グループ・ディスカッション(FGD)では、阻害因子と促進因子について検討した。結果の統合には、患者安全フレームワークを適用した。

結果

病院 3,026 施設中計 575 施設(19%)が自己評価質問票に回答し、このうち 516 施設(89.7%)が正式な ASP を有しており(中央値 4、四分位範囲[IQR]1 ~ 5 年)、14 施設が FGD に参加した。全 ASP 開発スコアの中央値は 48.4%(35.9 ~ 62.5%)であり、病院 41 施設(8.0%)が「不十分(0 ~ 25%)、237 施設(45.9%)が「基本的」(26 ~ 50%)、179 施設(34.7%)が「中間」(51 ~ 75%)、および 59 施設(11.4%)が「高度」(76 ~ 100%)に分類された。スコアが最も高かったのは、病院リーダーシップサポート(83.4%、IQR 66.7 ~ 100%)、次いで ASP チームおよび感染症トレーニング(66.7%、IQR 55.6 ~ 77.8%)、教育(50%、IQR 0.0 ~ 75.0%)、ASP 介入(43.8%、IQR 18.7 ~ 68.7%)、病院インフラ(42.9%、IQR 14.3 ~ 71.4%)、ならびにモニタリング、報告およびフィードバック(40.9%、IQR 27.3 ~ 54.5%)であった。ASP 開発スコアが高いことは、病院階層レベルが高いこと、ASP 実施期間が長いこと、ならびに地域レベルの 公衆衛生開発指数および一人当たり国内支出が高いことと関連したが、病院の所有権(公立・私立の違い)または地理的地域とは関連しなかった。FGD では、病院リーダーシップサポート、スタッフの技術的および行動的スキル、診療科間の協力、処方者の自律性が失われることへのおそれ、微生物学および IT サポート、ならびに病院認定に関連する阻害要因に焦点があてられた。

結論

特定された実施上の阻害因子は、ASP における背景に応じた、持続可能な改善を目指す取り組みに対して情報を提供することができる。
