Prescription of oral antibiotics and its appropriateness for outpatients in a tertiary care hospital in Korea

目的

抗菌薬適正使用支援プログラム（ASP）は注目を集めてきており、適切な抗菌薬使用の重要性に関する意識が高まっている。しかし、外来患者に対する抗菌薬処方を対象とした ASP の実施は、特に韓国ではまれである。本研究は、韓国の 3 次病院における外来患者に対する抗菌薬処方のパターンと適切性について分析することを目的とした。

方法

本研究は、Seoul National University Bundang Hospital の外来部門において、2018 年6 月 1 日から 2023 年 5 月 31 日にかけて経口抗菌薬処方のパターンを分析した。2023 年 5 月 15 日から 19 日のあいだに発行された処方箋の適切性を評価した。評価基準には以下を含めた：抗菌薬使用の適応、抗菌薬の選択、期間、および用量／頻度。薬剤師と感染症専門家が評価を行った。

結果

5 年間の研究期間中に外来受診計 7,282,407 件が記録され、経口抗菌薬は 243,967 件（3.4％）で処方された。抗菌薬処方の頻度は歯科、皮膚科および泌尿器科部門で最も高かった。最も多く処方された抗菌薬はセファロスポリン系薬、ペニシリン系薬およびスルホンアミド系薬であった。評価対象とした処方 423 件中、289 件（68.3％）および 134 件（31.7％）がそれぞれ治療目的および予防目的であった。処方のうち、治療目的では28.4％（289 件中 82 件）が、予防目的では 70.9％（134 件中 95 件）が不適切に分類された。不適切であった主な理由は、治療処方および予防処方のいずれにおいても不適切な適応であり、治療処方では 46.3％（82 件中 38 件）、予防処方では 96.8％（95 件中 92 件）を占めた。

結論

抗菌薬は、全外来受診の 3.4％で処方されていた。処方は、治療処方の 28.4％、予防処方の 70.9％で不適切であった。外来環境では、薬剤師による先を見越した広範な ASP 活動を検討すべきである。

外来における抗菌薬適正使用は大きな課題であるが、具体的な評価方法についての参考資料は限られている。本研究は、大学病院において外来患者を対象に、抗菌薬の適応、選択、投与期間、用量・頻度を精査し、不適切処方が治療で 28.4％、予防で 70.9％に上ることを明らかにした。特に予防的使用における適応不適切が主要因とされ、電子カルテを用いた膨大なデータ収集と詳細な分析には多大な労力が割かれた。外来での抗菌薬管理に向けた実践的な指針として、有用な資料と考える。
