Evaluating antimicrobial utilization in 20 Korean long-term care hospitals: a call to action for antimicrobial stewardship

背景

全国的な抗菌薬適正使用支援の達成のために、病院固有の抗菌薬使用の評価が必要である。本研究の目的は、韓国の長期ケア病院における抗菌薬使用について確認することである。

方法

韓国の長期ケア病院 20 施設における抗菌薬の処方パターンならびに適切性を評価するために、多施設後向き研究を実施した。各病院で抗菌薬を新たに処方された入院患者の医療記録データを 2023 年 7 月 10 日から 10 月 31 日まで手作業で収集し、抗菌薬使用の適切性を評価した。

結果

抗菌薬処方率は、2023 年 7 月 12 日時点で 8.9％（4,086 例中 365 例）、2023 年 10 月 18 日時点で 10.3％（3,892 例中 402 例）であった。全体で、患者 740 例に対して抗菌薬処方数は 885 件であった。抗菌薬のうちもっともよく処方されたクラスは第 3 世代または第 4 世代セファロスポリン系薬（31.9％）であった。大部分の抗菌薬（96.6％、885 件中 855 件）が感染症の治療のために処方されたが、感染症に対して適切に処方された抗菌薬は 37.7％（855 件中 322 件）にとどまった。投与経路、用量、処方された抗菌薬について適切であったのは、それぞれ 99.6％（855 件中 852 件）、56.1％（855 件中 480 件）、62.0％（855 件中 530 件）であった。全体で、患者の 35.2％（715 例中 252 例）が抗菌薬を適切に処方されていた。感染症の診断は、症例の 52.9％（892 例中 472 例）で適切であった。手術部位感染症予防、医学的予防、ならびに他の目的のために処方された抗菌薬、それぞれ 5 件、15 件、10 件のうち適切に使用されたものはなかった。

結論

韓国の長期ケア病院において、適切に使用された抗菌薬の割合は低い。これらのデータから、長期ケア病院における抗菌薬適正使用支援を構築する重要性が浮き彫りにされた。

韓国の複数の長期ケア病院 20 施設において、患者に対し適切に使用された抗菌薬の割合が低いことが明らかとされた論文である。処方された抗菌薬が目標とする原因菌が必ず見つかる訳ではないが、適正使用の原理・原則に見合った判断基準をよく考える必要がある。
