Determinants of compliance with infection prevention measures by physicians: a scoping review

感染予防策の適用により医療関連感染症が減少しうることを示すエビデンスにもかかわらず、これらの予防策の遵守率は、とくに医師において低い。介入の効果は持続しない場合が多い。医師の感染予防行動および行動変容戦略の成功を決定づける因子の全体像は欠けている。本レビューの目的は、どの決定因子が医師の感染予防行動に影響を及ぼすのか、遵守を改善するためにどの戦略が検討されているのか、これらの研究において実装科学の理論、モデルおよびフレームワークは用いられているかを明らかにすることであった。医療情報の専門家との協力により、PubMed、Embase、APA PsycInfo および Web of Science において 2023 年 6月 2 日までの文献検索を実施した。高所得国の医師の感染予防行動に焦点を当てたあらゆる種類の研究を対象とした。決定因子および戦略に関するデータを抽出し、決定因子を理論的ドメインのフレームワーク（Theoretical Domains Framework：TDF）に分類した。論文 56 報を組み入れた。TDF ドメイン「環境的背景と資源」、「社会的影響」、「結果についての信念」、「記憶、注意および意思決定」、「知識」および「技能」が最も重要であることがわかった。優勢な決定因子は、TDF 外のテーマである社会人口学的因子に対応している。持続可能な介入は、少なくともフィードバック、教育および擁護を含む集学的なアプローチである。理論、モデルおよびフレームワークが実装戦略の策定の指針として用いられることはまれである。結論として、介入研究では、対応を目指す決定因子が明確にされることはまれであり、理論的基礎が欠けていることがわかった。今後の取り組みにおいて、決定因子に関する知識を実装科学と組み合わせ、決定因子に合わせた理論に基づく介入を策定すべきである。

医師は医療従事者のなかでも感染対策の遵守率が低いことが過去から指摘され続けているにもかかわらず、医師特有の要因があり、これに対するアプローチが不十分であることは否めない。医師は感染対策の実施において、科学的エビデンスを重視する傾向が強い、多数の患者に接するとともに病棟間を頻繁に移動する、業務と対策とのバランスを考え業務遅延を嫌い時間的制約がある、緊急的処置と感染対策実施との間で優先順位も決定する、医療チームのリーダーである、専門性が高く自律性があるため感染対策への行動変容を受け入れにくい、などの医師の特性があり、これらを考慮した上でのアプローチが必要となる。
