医療機関の品質管理の取り組み：価値に基づく支払い制度が医療スタッフおよび医療関連感染症に及ぼす影響★

背景
韓国では、ケアの質を向上させるために、様々な品質評価が導入されている。そのため、医療機関の質の全体的なレベルは向上している。しかし、医療機関間には依然として格差が存在する。
目的
医療機関の品質管理の取り組みが、医療スタッフの確保および集中治療室（ICU）における医療関連感染症に及ぼす影響を評価した。
方法
本研究では、健康保険審査評価院が実施した 2 回目および 3 回目の ICU の質評価による、病院 265 施設と入院患者 39,096 例を含むデータを用いた。医療機関の継続的な品質改善の取り組みを、品質評価の結果に基づくグレードの変化に従って評価した。さらに、人工呼吸器関連肺炎およびカテーテル関連感染症の発生率を含む、医療関連感染症発生率を評価した。病院および患者の特徴を組み込んだ一般化推定方程式ポアソン回帰モデルを用いて、発生率比（IRR）を算出した。
結果
医療関連感染症は、人工呼吸器またはカテーテルを使用している患者の約 2％に発生した。人工呼吸器関連肺炎は、グレードが低下した医療機関で有意に増加した（IRR 2.038、95％信頼区間［CI］1.426 ～ 2.915）。グレードが改善した医療機関では、中心静脈カテーテル（IRR 0.484、95％CI 0.330 ～ 0.711）および尿路カテーテル（IRR 0.587、95％CI 0.398 ～ 0.866）に関連する感染症が減少した。
結論
韓国の ICU では品質評価が導入されているが、医療機関の間にある程度の格差が残っている。医療機関の品質管理の取り組みを通した医療資源の確保に差が認められ、感染率は管理の質が高い病院において低く、管理の質が低い病院において高かった。
監訳者コメント ：
「ICU の医療の質改善の取り組みが医療関連感染を減少させる」というシンプルではあるが、医療の質改善の重要性がわかる研究である。
