実臨床における 405 nm の青紫色光照射による患者のトイレの表面のコンタミネーションの除去★

背景
405 ~ 450 nm の範囲の青紫色光の照射は、病原性細菌に対して有効であることが報告されてきた。
目的
スウェーデンの 1 つの病院の 2 つの病棟の 7 つの患者用共用トイレにおいて、青紫色光の照射が表面のコンタミネーションの程度を減少させられるかどうかを調べること。
方法
405 nm の発光ダイオードスポットライトが取り付けられていたトイレで、5 カ所の別々の表面(ドアの取っ手、水道の取っ手、床、トイレのシート、トイレのひじ掛け)から、繰り返し標本を採取した。クロスオーバーデザインの前向き試験を実施した。試験には、2 つの試験期間が含まれ、最初は、スポットライトのスイッチがオンまたはオフで、次の試験期間は、スポットライトの状態が逆であった。
結果
試験中、合計で、665 個の表面標本が採取された(表面 1 カ所当たり 133 個の標本)。全標本の 84%で細菌の増殖がみられた。最もよくみられた結果は、コアグラーゼ陰性ブドウ球菌(staphylococci)とバチルス(Bacillus)属であった。全表面のコロニー形成単位(cfu)/m2 の中央値は、15(四分位範囲、5 ~ 40)であった。我々の主要転帰であるトイレの全表面の平均 cfu/m2 には、青紫色光の有無による差が観察されなかった。清潔な表面(< 5 cfu/cm2)は、交絡因子について調整したときも、青紫色光のあったトイレの方で多く観察された。ひどく汚染された表面の数に差は観察されなかった。
結論
この試験では、患者用共用トイレの通常の清掃に青紫色光を追加したときに、表面の細菌のレベルに対する相加効果を間違いなく示すことはできなかった。
監訳者コメント :
人が行う環境整備は不確実性があり、環境表面が細菌のリザーバーになる可能性があることから、近年、非接触式の環境消毒が注目されている。青紫色光照射もそのひとつである。過酸化水素蒸気や紫外線(UV)照射では細菌に有効である一方で、人体や材料に悪影響を与える可能性がある。青紫色光照射は細菌に有効であるものの、UV 照射より殺菌力が低く、人体に重大な悪影響を及ぼさない。今回の研究では青紫色光照射でのトイレの環境整備の有効性は示せなかったが、安全性の高い非接触式環境消毒であり、今後の効果的な活用方法の実証が期待される。
