血液病棟および他の高リスク臨床環境における菌血症リスク患者の特定に、緑膿菌（Pseudomonas aeruginosa）の鼠径部／直腸内保菌スクリーニングは有用か？★

背景
病院の水に曝露される患者において、緑膿菌（Pseudomonas aeruginosa）は医療関連感染症の主な原因である。当該 3 次教育病院における緑膿菌菌血症の発生率の上昇傾向により、介入が促された。
目的
患者入院時に微生物学的スクリーニングを実施し、感染の早期発見を支援すること。
方法
病床数 41 床の血液病棟（ロンドンの 800 床の教育病院）を 2020 年 1 月 24 日から 2020 年 5 月 13 日まで調査した。入院時および週 1 回、直腸と鼠径部のスワブ検体を同時に重複して採取した。結果を、シャワー、排水管、水道水のこれまでの汚染データと比較した。
結果
患者 154 例（女性 61 例、男性 93 例）から鼠径部／直腸サンプル計 606 個を採取した。入院した 154 例中 6 例（3.9％）が緑膿菌陽性であった。2 例（1.3％；95％信頼区間［CI］0.16 ～ 4.6）は、入院時に保菌していたが、4 例（2.6％; 95％CI 0.7 ～ 6.5）は、保菌前入院期間の中央値 33 日（四分位範囲 13 ～ 54）までに保菌状態となった。同時の重複サンプリングでは、すべての保菌患者において、陽性と陰性の両方の結果が示された。その後、1 例は緑膿菌菌血症を発症した。緑膿菌が検出された患者 4 例の病室のシャワー水と対応する排水管は、入院の 265 日前（中央値 247 ～ 283）に緑膿菌の濃厚汚染（＞ 300 cfu/100 mL）を受けていた。
結論
入院時の直腸／鼠径部スクリーニングは、患者保菌の早期発見に有益かもしれないが、侵襲的であり、資源を要し、成果が低い傾向にある。高リスク環境において、とくに入院期間が 30日を超すと予測される場合、環境モニタリングの強化、表面および排水管の汚染除去、ポイントオブユースのフィルターバリアを推奨する。
監訳者コメント ：
血液病棟などにおける菌血症リスク患者の特定に、緑膿菌（Pseudomonas aeruginosa）の鼠径部／直腸内保菌スクリーニングは、成果が低い傾向にあり、臨床では環境検査のモニタリングをルーチンにまず実施すべきだろう。
