新生児における中心静脈ライン関連血流感染症を減少させるための新しい多腔型輸液装置の費用対効果★

背景
薬物配合禁忌や中心静脈ライン関連血流感染症（CLABSI）に関係する問題に対処することを目的として、新生児集中治療室（NICU）でのマルチ輸液用の新しい医療機器が開発された。
目的
この新しい灌流システムを NICU の環境で実施することの費用対効果を評価すること。
方法
新しい灌流システムの実施前（2019 年）と実施後（2020 年）の費用および有効性を比較するために、生後 3 日以内に NICU に入室し、中心静脈ラインを必要としたすべての乳児を対象に、この単一施設観察研究を実施した。費用は、病院の視点から算出し、CLABSI の発生率は、NICU 入室から退室までの期間を対象に調査した。資源の利用を測定し（輸液装置、感染症治療薬、生物学的解析）、対応する費用を、2019 年の料金表を用いて見積もった。増分費用効果比を算出し、回避されたCLABSI 1 件当たりのユーロで表わした。そして、一元および多変量感度分析を実施した。
結果
選択された 609 例の乳児において、臨床特性は、両方の期間で類似していた。新しい灌流システムの実施後、CLABSI の発生率が有意に減少し（率比 0.22、95％信頼区間 0.07 ～ 0.56）、総費用が 1,000 カテーテル日当たり €65,666 から €63,932 まで減少した（P ＜ 0.001）結果、増分費用効果比は回避された CLABSI 1 件当たり €251 となった。感度分析の大多数は、新しい介入が依然として経済的に優位であったことを示した。
結論
この単一施設研究で、新しい灌流システムの実施後、CLABSI の発生率が有意に減少し、追加の費用は発生しなかったことが示された。更に、前向き多施設共同無作為化試験を行い、これらの結果を他の NICU で確認することが必要である。
監訳者コメント ：
CLABSI 予防のため、新しい輸液装置の臨床研究が必要である。
