背景

関節全置換術中の人工関節感染(PJI)の予防に、グルコン酸クロルヘキシジン(CHG)とポビドンヨードがよく使われるが、それらの効果的な濃度や、バイオフィルムへの影響については、あまりはっきりしていない。

目的

(1)PJI を引き起こすモデル微生物および臨床分離株に対する CHG およびポビドンヨードの in vitro の最小発育阻止濃度、(2)CHG およびポビドンヨードのバイオフィルム形成に対する影響、(3)2 つの溶液を組み合わせると相乗的な利益があるかどうか、(4)抗菌薬であるバンコマイシンを追加することが殺菌作用に影響を与えるかどうか、を明らかにすること。

方法

我々は、濃度を漸増させた CHG および/またはポビドンヨードの存在下で、表皮ブドウ球菌(Staphylococcus epidermidis)、メチシリン感受性およびメチシリン耐性黄色ブドウ球菌Staphylococcus aureus)、大腸菌(Escherichia coli)、緑膿菌(Pseudomonas aeruginosa)、カンジダ・アルビカンス(Candida albicans)、最近の臨床分離株の in vitro での増殖とバイオフィルム形成を測定した。チェッカーボード法を使い、両溶液の相乗効果とバンコマイシンとの相乗効果の可能性を測定した。

結果

CHG とポビドンヨードは、試験したすべてのモデル微生物の増殖とバイオフィルム形成を、それぞれ、0.0004%と 0.33%以下の濃度で阻害した。高度に希釈した濃度では、逆説的に、バイオフィルム形成が増加した。これらの溶液の間に相乗効果はなく、バンコマイシンとは独立に作用した。

結論

CHG とポビドンヨードは、通常使われるより低い濃度でも効果があり、創傷の消毒を最適化することを目的とした更なる臨床試験を支持するベースラインを確立している。両方を併用することによる相乗的な利点はない。バンコマイシンは、表皮ブドウ球菌と黄色ブドウ球菌の増殖を阻止する効果がある。しかし、バンコマイシンは、緑膿菌のバイオフィルムの産生を刺激するので、緑膿菌による PJI の稀な症例では、感染を悪化させる可能性があることが示唆される。

監訳者コメント

日常的に使用されている消毒薬である CHG とポビドンヨードについて、手術で使用されるよりも低い濃度で抗菌活性、抗バイオフィルム活性を有すること、逆に高い濃度でバイオフィルムを形成しやすいこと、CHG とポビドンヨードを併用しても相乗作用や拮抗作用を示さないこと、バンコマイシンとこれらの消毒剤を併用しても相乗作用や拮抗作用を示さないこと、バンコマイシンは緑膿菌のバイオフィルム産生を刺激し、感染を悪化させる可能性があるなど、本研究は非常に新知見に富んでいる。是非読んでいただくことをおすすめする。
