呼吸器疾患の患者を空間的に分離することが、救急部門のフロープロセス間隔と滞在期間に与える影響★

背景

救急部門(ED)における空間的分離は、経路別予防策の一環として、経験的に実践されている。感染させる可能性がある患者を隔離することで利益がある可能性があるにもかかわらず、空間的分離が患者のフローに対する影響は、いまだ不明である。

目的

ED の患者のフローに対する空間的分離の影響を調査し、ED 滞在期間に影響を与える特異的な臨床因子およびフロープロセス間隔を特定すること。

方法

これは、2022 年の 1 月 1 日から 3 月 31 日までの患者の電子カルテから抽出したデータの後向き研究で、マレーシアのクアラルンプールにある 3 次病院の ED で実施した。この期間中、患者は、米国疾病対策センター(CDC)の推奨に基づき、呼吸器区域と非呼吸器区域に分離された。研究は、施設の倫理委員会から倫理面の承認を得た。

結果

合計で 1,054 例の患者が研究に組み入れられ、275 例が呼吸器区域、779 例が非呼吸器区域に割り当てられた。呼吸器区域の患者は、非呼吸器区域と比べて、ED 滞在期間の中央値が有意に長かった(9 時間 29 分対 7 時間 6 分、P < 0.001、d = 0.41)。ED 滞在期間が 8 時間以下を達成した患者の割合は、呼吸器区域の方が非呼吸器区域と比べて低かった(41.8%対 58.3%、P < 0.001)。ED 滞在期間に影響を与えた独立因子は、トリアージのカテゴリー、再トリアージ、高血圧、バイオメディカルイメージングの実施、内科・外科・救命救急医療への相談、患者処遇計画であった。ED 滞在期間を有意に延長した障害は、ED の退院の決定、超音波検査の間隔、紹介から診察までの間隔であった。

結論

空間的分離は、フロープロセス間隔と ED 滞在期間を延長した。入院への障害となるものに取り組むことと、専門医による検討とバイオメディカルイメージングの過程を簡素化することによって、呼吸器区域の ED 滞在期間を短縮できる可能性がある。

監訳者コメント

呼吸器感染症の伝播には空間的拡散が関与しており、呼吸器感染症が疑われる患者の空間分離戦略は、疾患の拡散を緩和し、医療従事者を曝露から守るための潜在的な解決策となりうる。しかし、空間的分離は陰圧環境や高効率空気ろ過システム、個人防護具の着用、画像機器を含むすべての接触面の消毒など準備に追加の時間を割り当てる必要があり、患者フローに影響がでることが懸念される。本研究では救急部門での空間分離において、患者の滞在時間に影響を与えるものを明確にしており、ややもすると非効率な隔離予防策の中で、救急部門の滞在時間の短縮(効率化)の両立させようとしている点が興味深い。
