排水管関連アウトブレイクに関する記述的レビューおよび最新情報★

背景
病院排水システムに関連したアウトブレイクはよく報告され、依然としてインシデント管理チームに課題を提示している。そのようなアウトブレイクは長期化、複雑化することがあり、改善には集学的な戦略が求められる。
目的
最近の排水管関連アウトブレイクの概要を示し、集学的な制御策が実施されているかどうかを調査し、先行因子を検討した。
方法
データベースにおいて、5 年間の排水管関連アウトブレイクを検索した。用いた検索語は、「healthcare drainage outbreaks（医療 排水管 アウトブレイク）」、「drain outbreaks（排水管 アウトブレイク）」、「drainage system outbreaks（排水 システム アウトブレイク）」、「sink outbreaks（シンク アウトブレイク）」、「shower outbreaks（シャワー アウトブレイク）」などであった。発生国、関与する病原体、発生した病棟、排水管の種類、患者数、排水システムへの介入、排水管消毒薬の種類、感染制御の介入、タイピングの方法、帰結および先行因子に関する情報を収集した。
結果
19 件の排水管関連アウトブレイクに関する研究をレビューした。インシデントの大半がカルバペネマーゼ産生腸内細菌目細菌に起因し、救命救急環境で発生していた。ほとんど（19 件中 16 件）の研究において、集学的アプローチの必要性が認識されていた。介入の成功に関する情報はすべてのインシデントで記録されたわけではなかったが、19 件中 13 件の研究で、制御策実施後にそれ以上の症例はなかったことが報告された。排水管消毒に関して、薬剤の選択および適用の頻度にばらつきが認められる。7 件の研究で先行因子が考察されていた。
結論
排水管関連アウトブレイクは過去 24 年間報告があり、このテーマに関するレビュー論文があるにもかかわらず、依然として重大な課題をもたらしている。現時点では、排水管関連アウトブレイクの管理や、リスクを軽減するための新たな建物の設計に関する英国の指針は存在しない。病院排水システムからの課題への対処は、当局および指針作成者によって優先事項とみなされるべきである。
監訳者コメント ：
病院排水システムは汚染菌が流失すると、病院環境に重大な影響を及ぼす。定期的な検査によるモニタリングと『排水処理』が重要である。
