背景
整形外科手術後の切開創の手術部位感染症(SSI)を含めて、病院感染に季節性があることが認められているが、一般外科および消化器外科手術後の切開創の SSI に季節性があるかどうかは、依然としてはっきりしていない。
目的
一般外科および消化器外科手術後の切開創の SSI の季節性とリスク因子を解析すること。
方法
これは、単変量および多変量解析を使った後向き単一施設観察研究であった。評価した変数は、年齢、性別、手術方式、手術の緊急性、手術時間、創傷の分類、米国麻酔学会(ASA)全身状態、等であった。
結果
合計で 8,436 例の患者が登録された。一般外科手術(N = 2,241)では、夏が(3.9%、オッズ比[OR]1.87、95%信頼区間[CI]1.05 ~ 3.27、P = 0.025)、その他の季節と比べて(2.1%)、顕著な SSI の発生率を示した。逆に、消化器外科手術(N = 6,195)では、冬の発生率が(8.3%、OR 1.38、95%CI 1.10 ~ 1.73、P = 0.005)その他の季節より(6.1%)高かった。一般外科手術について夏(OR 1.90、95%CI 1.12 ~ 3.24、P = 0.018)と、消化器外科手術についての冬(1.46、1.17 ~ 1.82、P = 0.001)が、切開創の SSI の独立したリスク因子として浮かび上がった。直視下手術(OR 2.72、95%CI 1.73 ~ 4.29、P < 0.001)と ASA 全身状態スコアが 3 以上であること(1.64、1.08 ~ 2.50、P = 0.021)が、冬の間に消化器外科手術を受ける患者における切開創の SSI の独立したリスク因子であった。
結論
一般外科および消化器外科手術後の切開創の SSI の発生率には季節性が存在する。これらの傾向を認識することは、予防戦略を増強するのに役立つ可能性があり、一般外科手術では夏、消化器外科手術では冬にリスクが上昇することを、はっきりと示している。
