背景

トイレから患者への多剤耐性緑膿菌の伝播を予防することは、多くの骨髄移植部門で、管理に関係した課題となっている。

目的

多剤耐性緑膿菌のトイレへの定着が持続している骨髄移植部門における、既存の感染制御対策の下での、トイレから患者への 多剤耐性緑膿菌の伝播率を、後向きに縦断的に解析すること。

方法

現地での感染防御バンドルは、(1)感染防御についての患者教育、(2)通常の患者スクリーニング、(3)トイレに流す水の量を 9 L とすること、(4)トイレの水のタンクの臭素化、(5)過酸化水素を使ったトイレの汚染の除去から成っていた。トイレの水は、2016 年から 2021 年までの間、定期的に標本が採取されていた(少なくとも 3 か月に 1 回で、26 回間隔を空けていた)。多剤耐性緑膿菌が検出されたら、3 cfu/100 mL 以下に達するまで、消毒と標本の再採取が繰り返された。入手できた 多剤耐性緑膿菌の分離株すべてについて、後向きに、全ゲノムシークエンシング(WGS)を実施した(陽性の環境サンプル 117 個のうち 90 個、患者 14 例のうち 10 例で、9 件の院内感染を含む)。

結果

患者の分離株の WGS では、6 つの配列型(ST)が同定され、ST235/CT1352/FIM-1 と ST309/CT3049/カルバペネマーゼなしが多かった(それぞれ単離株が 3 株)。環境からの標本採取では、一貫して、多剤耐性緑膿菌 ST235 が同定され(65.5%が ST235/CT1352/FIM-1)、遺伝的多様性が低いことを示した(コアゲノム複数部位塩基配列タイピング[cgMLST]で差が 29 アレル以下)。これは、多剤耐性緑膿菌は広く行き渡っているけれども(79 の機会に検出、すべてのトイレで検出)、トイレから患者への直接の伝播は稀であったことを示している。6 年間で、ST235/CT1352/FIM-1 であるトイレの 多剤耐性緑膿菌集団が原因と考えられる多剤耐性緑膿菌患者分離株は、3 株しかなかった。

結論

厳格な標的を絞ったトイレの消毒によって、患者が多剤耐性緑膿菌に感染するリスクの可能性を減少させることができる。

監訳者コメント

骨髄移植部門では多剤耐性緑膿菌などのグラム陰性菌の伝播が問題となるが、トイレでの感染防御バンドルを全ゲノムシークエンシング(WGS)で評価した結果、トイレでの多剤耐性緑膿菌の伝播率が低いことがわかり、バンドルによる戦略が成功していることを明らかにしている。
