周期的に蓄積するバイオフィルムを除去することを目的とした内視鏡チャネルの 2 つの洗浄法の比較★

序論
バイオフィルムは、内視鏡チャネルに細菌が存在し続けることに大きく寄与している。内視鏡のすべてのチャネルからバイオフィルムを除去することができる強化された洗浄法が、患者への感染リスクを減少させるのに必要である。この試験では、周期的に蓄積するバイオフィルムの除去を、ポリテトラフルオロエチレンチャネルを対象に、自動化された内視鏡チャネル洗浄機と、使用説明書に従った標準的な手動洗浄を、直接比較した。
方法
周期的に蓄積するバイオフィルムを、内径が 1.4 mm(空気/水および補助チャネルを表す)と 3.7 mm(吸引/生検チャネルを表す)のポリテトラフルオロエチレンチャネルで増殖させた。すべてのチャネルを、残留総有機炭素、蛋白質、生菌について検査した。国際的に認められている ISO 15883-5:2021 警戒基準を、蛋白質(3 μg/cm2)と全有機炭素(6 μg/cm2)の洗浄の基準として用いた。
結果
自動洗浄機は、空気/水/補助チャネルを表す 1.4 mm のチャネルと、吸引/生検チャネルを表す 3.7 mm のチャネルで評価したすべてのマーカー(蛋白質、総有機炭素、生菌)において、手動洗浄より、性能が有意に優れていた。手動洗浄では、空気/水および補助チャネルからバイオフィルムを除去することができなかった。使用説明書では、これらのチャネルはブラッシングされないため、文献で報告されている内視鏡に関連した多くの感染の一部は、これが根本的な原因の 1 つである可能性が示唆される。
結論
自動化された内視鏡チャネル洗浄機は、すべての内視鏡チャネルにおいて、現在のやり方より強化された洗浄法である可能性があるので、感染リスクに対処できるかもしれない。
監訳者コメント :
日本では欧米と異なり、小規模クリニックにおいても広く消化器内視鏡検査・治療が行われているが、消化器内視鏡の洗浄・消毒の標準化がなされているとは言い難い。幸いにして、欧米より耐性グラム陰性菌の頻度は高くないが、今後問題になっていく可能性があり、内視鏡を介した耐性グラム陰性菌の伝播の防止はより重要になる。空気/水/補助チャネルに関連する感染は盲点で、今後の課題のひとつになるだろう。
