集中治療室(ICU)におけるアウトブレイクの潜在的な発生源であるシンクおよび多剤耐性(MDR)菌伝播に対する認識が向上し、水を使用しない患者ケアシステムへの関心が高まっている。本システマティックレビューでは、ICU 環境における水を使用しない実践の有無にかかわらず、水系医療関連感染症の減少を目的としたシンク撤去の効果に関する入手可能なエビデンスを調査し、統合した。1980 年 1 月 1 日から 2024 年 4 月 2 日までに発表され、医療関連感染症と患者保菌を減らすことを目的に、ICU において水を必要としない実践または水を使用しない実践を検討した研究を、5 つのデータベース(PubMed、MEDLINE、Scopus、Web of Science、Embase)を用いて検索した。論文 2,075 報のうち 7 報の準実験的研究(計 332 病床)が、研究の選択基準を満たした。これらの研究 7 報のうち 6 報(85.7%)は成人 ICU、1 報(14%)は新生児 ICU におけるものであった。7 施設中 5施設(71.4%)が、アウトブレイク後に水を使用しない方法での介入を実施した。水を使用しない代替法には、水を必要としない入浴剤(7 施設中 6 施設、85.7%)、ボトル入り飲料水(7 施設中 3施設、42.9%)、口腔ケア(7 施設中 3施設、42.9%)、経口薬の溶解(7 施設中 4施設、57.1%)、患者および調剤エリア外に指定された廃水処理用「汚染」シンク(7 施設中 4 施設、57.1%)などが含まれた。研究の対象となった病原体は、MDR グラム陰性菌(7 施設中 4施設、57.1%)、MDR 緑膿菌(Pseudomonas aeruginosa)のみ(7 施設中 2施設、28.6%)、肺非結核性抗酸菌(7 施設中 1 施設、14.3%)であった。7 施設中 5 施設(71.4%)の研究では、アウトブレイクの終息が報告された。、限られた件数の研究(その大多数がアウトブレイク状況下での実施)からの予備的エビデンスでは、ICU におけるシンク撤去と水を使用しない他の介入は、蛇口が関与するアウトブレイクの終息、および院内における肺非結核性抗酸菌の呼吸器分離株の減少に寄与することが示唆された。
