背景

COVID-19 パンデミックと、多剤耐性微生物による侵襲性感染症の発生率との関連は、依然として議論のテーマとなっている。

目的

それぞれ異なる抗菌薬耐性パターンが認められ、COVID-19 パンデミックによって異なる影響を受けたスイスの 2 つの地域において、広域スペクトルセファロスポリン耐性を有する大腸菌(Escherichia coli)および肺炎桿菌(Klebsiella pneumoniae)による血流感染症(BSI)の国内発生率について分析すること。

方法

2015 年 1 月 1 日から 2022 年 8 月 31 日にかけて全国サーベイランスシステム(ANRESIS)で前向きに収集された陽性血液培養を用いてデータを解析した。COVID-19 患者の占有率と広域スペクトルセファロスポリン耐性発現率との間に考え得る関係を探索するため、2020 年 4 月 1 日から 2022 年 3 月 30 日の 2 年間にわたるパンデミック期間について、疑似 Poisson 解析およびロジスティック回帰分析を用いて詳細な解析を実施した。

結果

本研究期間中に、大腸菌 BSI エピソード 40,997 件および肺炎桿菌 BSI エピソード 8,537 件が収集され、参加病院から ANRESIS に報告された。広域スペクトルセファロスポリン耐性は、大腸菌の 11%(N = 4,313)および肺炎桿菌の 8%(N = 664)で認められた。COVID-19 発生率が高いほうの地域では、パンデミック時に広域スペクトルセファロスポリン耐性大腸菌 BSI の発生率に有意な低下が認められた。対照的に、パンデミック時には両地域において、広域スペクトルセファロスポリン耐性肺炎桿菌 BSI 発生率が最初大幅に低下した後に上昇したが、この効果は統計学的に有意ではなかった。COVID-19 患者による病院占有率と上記の傾向との間には関連は認められなかった。

結論

COVID-19 パンデミックの早期には、広域スペクトルセファロスポリン耐性の発現率の低下がみられ、とくに COVID-19 パンデミックによる影響が最も大きかった地域では広域スペクトルセファロスポリン耐性大腸菌 BSI の発生率が低下した。
