背景

臨床区域における換気グリルの環境サーベイランスについて、これまでに系統的な解析は行われていない。

方法

内科病棟 6 棟および外科病棟 11 棟において、頻繁に手が触れる物品(N = 529)、給気グリル(N = 295)および排気グリル(N = 184)から検体を採取し、多剤耐性菌(MDRO)としてメチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)(MRSA)、カルバペネム耐性アシネトバクター・バウマニー(Acinetobacter baumannii)(CRAB)およびカルバペネマーゼ産生腸内細菌目細菌(CPE)の培養を行い、分離株を選択して全ゲノムシークエンシング(WGS)を行った。汚染率と、各 MDRO の定着圧との相関性を検討した。

結果

2023 年 10 月 3 日から 11 月 21 日までに、9.8%(99/1,008)の検体が検査陽性となり、検出された MDRO は MRSA(24.2%、24/99)、CRAB(59.6%、59/99)および CPE(2.0%、2/99)のみであった。汚染率は排気グリル(26.6%、49/184)のほうが給気グリル(5.8%、17/295)より有意に高かった(P < 0.001)。いずれかの MDRO による排気グリルの汚染率は、急性期内科病棟(73.7%、14/19)において外科病棟(12.5%、4/32)より有意に高かった(P < 0.001)。しかし、コホートキュービクルの内側と外側に位置する排気グリルの間で、MDRO による汚染率に差は認められなかった(27.1%、13/48 対 28.8%、30/104、P = 0.823)。それでも、週ごとの CRAB 定着圧と、全体の環境汚染率との間(r = 0.878、95%信頼区間[CI]0.136 ~ 0.986、P = 0.004)、ならびに給気グリル(r = 0.960、95%CI 0.375 ~ 0.999、P < 0.001)および排気グリル(r = 0.850、95%CI 0.401 ~ 0.980、P = 0.008)の汚染率との間に、有意な相関が認められた。WGS によって、患者および排気グリルから採取された分離株のクローン関連性が示された。

結論

換気グリルは MDRO のリザーバとなる可能性がある。囲いのないキュービクルにおけるコホート看護では、空中飛散を介する MDRO 伝播が完全には防止されない可能性があることから、今後の病院設計の検討が促される。

監訳者コメント

環境や物品表面と空中にはある程度連続的に微生物が存在することは理論的には予想される。黄色ブドウ球菌など従来接触による伝播が主体であるとされている微生物について、エアロゾルなどを介した伝播や拡散をどの程度考慮しなければならないか、についてはまた次の課題ではあるが、まずはファクト(事実)を示した点で、重要な研究である。
