Factors associated with hand hygiene adherence among healthcare workers in Guatemala during the COVID-19 pandemic

背景

医療関連感染症は低および中所得国で常に発生しており、患者ケア時の適切な手指衛生遵守によって減少する可能性がある。

目的

擦式アルコール製剤(ABHR)を製造して、グアテマラの ケツァルテナンゴ にある公立 1 次および 2 次医療施設19施設に配布し、医療従事者の手指衛生遵守を評価するため、また手指衛生に関連する因子を特定するために手指衛生の観察を実施した。手指衛生遵守は、石けんと水による手洗い、あるいは ABHR を使用することと定義した。

方法

ABHR 配布の前(2021 年、ベースライン)および後(2022 年、フォローアップ)に観察を実施し、手指衛生実践の経時的な変化を評価した。2 値比較および混合効果ロジスティック回帰モデルを用いて、手指衛生遵守と以下の独立変数との関連を検討した:医療施設のレベル、実施された患者との接触の種類、手指衛生実施のタイミング、医療従事者の職種、および存在した材料(例、水、石けん、ABHR)。

結果

各観察期間に、医療従事者 67 名 および 82 名において、それぞれ患者との接触 243 件および 300 件を観察した。いずれの観察期間においても手指衛生遵守率は低かった(ベースラインで 40%、フォローアップで 35%)。医療従事者では、侵襲的接触時、患者との接触後、ならびに医師の場合に手指衛生遵守率が高かった。

結論

手指衛生遵守率はシナリオによってばらつきがあり、このことは、遵守率を改善するためには、行動変化をもたらす複数の決定因子に取り組む重要性を示している。このためには、多面的アプローチにより介入を実施することが必要であり、こうしたアプローチとして、手指衛生用の材料およびインフラの両方へのアクセスを高めること、ならびに手指衛生教育および訓練、モニタリングおよびフィードバック、リマインダー、ならびに手指衛生の安全文化を促進することなどが挙げられる。

監訳者コメント

中米のグアテマラからの手指衛生遵守の報告。手指衛生の遵守率は 40%以下と低く、WHO手指衛生多角的戦略において、グアテマラのような低中所得国ではまずはインフラなどの物品設備に大きな課題があることが理解できる。
