目的
クロルヘキシジングルコン酸塩(CHG)やオクテニジン二塩酸塩(OCT)などの消毒薬は、病院でメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus:MRSA)の伝播を予防および制御するために頻用されている。MRSA の CHG 感受性低下の割合が高まっていることから、OCT 感受性が低下する可能性が懸念されている。本研究では、3年間にわたり CHG 感受性低下および OCT 感受性低下の割合を評価するとともに、OCT への曝露と MRSA の OCT 感受性低下との間の関連を評価した。
方法
2019 年から 2021 年までに長期療養施設で MRSA に感染した入院患者から分離したMRSA 分離株を、消毒薬感受性検査の対象とした。2019 年 1 月から 9 月まで全入院患者に対する毎日の CHG 浴を行い、2019 年 10 月以降は全入院患者に対する毎日のOCT 浴を行った。CHG および OCT の 最小発育阻止濃度(MIC)を、微量液体希釈法を用いて測定した。OCT への曝露が OCT 感受性低下と独立して関連するかどうかを、多変量ロジスティック回帰分析を用いて評価した。
結果
186 分離株のうち 179 株(96%)で CHG 感受性が低下しており(MIC ≧ 4 mg/L)、46 株(25%)で OCT 感受性が低下していた(MIC ≧ 2 mg/L)。OCT 感受性低下の割合は、2019 年、2020 年および 2021 年で、それぞれ 26.9%、13.8%および 14.3%であった。CHG 感受性低下の割合は、2019 年、2020 年および 2021 年で、それぞれ 95.4%、100%および 95.9%であった。年齢、性別、人種、検体採取年、施設でのリスク日数、前年の入院および前年の MRA 保菌/感染で補正後に、OCT への曝露は OCT 感受性低下と関連していなかった(補正オッズ比[OR]0.23、95%信頼区間(CI)0.08 ~ 0.75、P = 0.014)。
結論
長期療養施設で全患者に対する OCT 浴を行ったにもかかわらず、OCT 感受性低下の割合は低いままであった。しかし、CHG 感受性低下の割合は高かった。OCT への曝露は、MRSA の OCT 感受性低下とは関連していなかった。
監訳者コメント :
CHG 浴と異なり、OCT 浴で MRSA のOCT 感受性低下を認めなかった。今後の長期療養施設の MRSA 対策の一助になるかもしれない。
