腸炎起因性メチシリン耐性黄色ブドウ球菌の分子疫学 ★

1990年代初期に、日本でメチシリン耐性黄色ブドウ球菌が原因の重度の腸炎(MRSA腸炎)が流行したが、それ以降、発生率は減少している。1990~1993年に検出された、MRSA腸炎の原因となった12の分離菌株(腸炎症例からの分離菌株)の遺伝子型および表現型を、1998~2002年に検出された186の非腸炎症例からの分離菌株と比較した。ブドウ球菌腸毒素(SE)および毒素性ショック症候群毒素(TSST-1)の産生を検索するため、パルスフィールド・ゲル電気泳動法(PFGE)、コアグラーゼ型別および逆受身ラテックス凝集反応により菌を検査した。また、SE-A、SE-B、SE-C、SE-D、TSST-1などの蛋白質をそれぞれコードする構造遺伝子entA、entB、entC、entD、tstを検索するため、PCR法により菌を検査した。12の腸炎症例からの分離菌株を、4つの型と4つの亜型に分類した。186の非腸炎症例からの分離菌株のうち、腸炎性症例からの分離菌株と区別できないPFGEパターンを示したのはわずか7株であった。12の腸炎症例からの分離菌株のうちの8株で、entA、entC、tstが認められ、高濃度のSE-AおよびTSST-1の産生がみられたが、SE-Cは産生していなかった。186の非腸炎症例からの分離菌株のうち、157株がSE-CおよびTSST-1を産生していたが、SE-Aは産生していなかった。腸炎患者からの分離菌株と区別できないPFGEパターンを示した非腸炎症例からの分離菌株7株はSE-Aを産生せず、比較的低濃度のTSST-1産生が認められた。これらの分離菌株が、集団発生早期から当院の病棟に存在し続けていた可能性があるが、異なる表現型を獲得していた。結論として、MRSA腸炎の消失は、腸炎起因性のクローン菌株の割合および表現型変異が減少した結果である可能性がある。

本論文は広島大学外科教室からの報告でありMRSA腸炎発症の病原因子の背景を知るうえで参考になる(MRSAに起因する感染性腸炎に関する海外での知見は乏しく、国内外の状況あるいはその認識は大きく異なっている)。
