心臓手術後の陰圧閉鎖療法の予防的使用：メタアナリシス★★

心臓手術患者における手術部位感染症（SSI）は頻度の高い合併症であり、患者の不快感の増大と入院期間の延長につながる。本メタアナリシスでは、心臓手術後の縫合された手術創における単回使用陰圧閉鎖療法（sNPWT）デバイスの予防的作用を評価することを目的とし、すべての心臓手術患者への予防適用の可能性について調査した。ScienceDirect において2000 年から 2022 年までに発表された関連する研究に焦点を当て、「negative pressure wound therapy（陰圧閉鎖療法）」または「PICO negative pressure wound therapy（PICO 陰圧閉鎖療法）」と「cardiac surgery（心臓手術）」または「sternotomy（胸骨切開術）」を組み合わせた包括的な文献検索を実施した。選択基準は、心臓手術縫合創に対して sNPWT を従来のドレッシング材と比較する症例対照研究で、胸骨正中切開術を受け、手術直後の感染合併症がなく、SSI に関する詳細が入手可能な成人患者を含むものとした。当該施設において sNPWT を受けた症例の後向き分析も実施した。メタアナリシスにより、sNPWT の予防的作用が示され、全 SSI のリスクが 44％低下（オッズ比［OR］0.56）、深部手術創感染のリスクが 40％低下した（OR 0.60）。しかしながら、表層手術創感染では予防効果は有意ではなかった。単一施設研究はメタアナリシスの結果と一致し、sNPWT の有効性を確認し、メタアナリシスに含めた。結論として、メタアナリシスと単一施設研究はともに、全 SSI および深部 SSI に対する陰圧閉鎖療法の予防的作用を支持するものであり、すべての心臓手術集団における予防的使用の可能性が示唆される。

心臓手術後の手術部位創傷感染に対する陰圧創傷療法は、その有効性が数多くの論文により、証明されている。
