背景

術後深部胸骨創感染症の形で現れる心臓手術後の手術部位感染症(SSI)は、稀だが、致死的となる可能性がある合併症の1つである。これへの対処では、SSI のリスク因子の大部分は制御できないために、予防策に焦点が当てられている。したがって、手術室には、創傷の空気による汚染を予防するための暖房・換気・空調システムが備え付けられている。システムは、乱流混合の気流、または一方向流型の気流を使用している。

目的

気流を乱流混合から一方向流型に変えたことで心臓手術後の SSI のリスクが減少したかを調べること。

方法

この観察的後向き単一施設コホート研究では、2 年間に亘って、開心術を受けた患者 1,288 例からデータを収集した。2 つの研究期間で、施設の SSI の予防策は同じままであったが、使われた暖房・換気・空調システムの型が違っていた。

結果

交絡因子(糖尿病、肥満、手術時間、再手術)を考慮した多変量ロジスティック回帰分析を使ったところ、乱流混合の気流は SSI の独立したリスク因子であるという仮説が棄却された(オッズ比 0.9、95%信頼区間 0.4 ~ 1.8、P > 0.05)。開心術を受けた患者の SSI の発生率に対する一方向流型の気流の予防効果を、乱流混合の気流と比べて証明することはできなかった。

結論

これらの結果に基づくと、開心術における一方向流型の気流の使用は、電力消費の多さのために、費用効果が低いことと、環境への有害な影響があることについて慎重に検討すべきである。心臓手術において、乱流混合の気流を利用してエネルギーの使い過ぎを減らすことで、手術室のカーボンフットプリントと、その気候関連の健康被害への寄与を減らせる可能性がある。

近年、カーボンニュートラルを実現するために温室効果ガスの排出削減を企業単位で進めているが、さらに製品単位で進めようというのがカーボンフットプリント(CFP)である。心臓手術における SSI 発生率に一方向型と乱流混合型の気流で比較した結果、有意差がなかったことから、手術室の空調システムにおける CFP の効果と、乱流による空調システムでの費用対効果に言及している。本研究は後向きの単一施設のコホートであり、よりエビデンスレベルの高い RCT による研究が期待される。
